A qualitative study on barriers and facilitators to exercise in patients receiving neoadjuvant chemotherapy for breast cancer

摘要: 目的:研究乳腺癌新辅助化疗患者化疗期间运动阻碍与促进因素,为制定运动干预计划、构建科学的运动干预方案提供参考依据。方法:采用现象学研究方法,选取2023年8月1日至10月31日于福建省某三级甲等肿瘤专科医院乳腺外科首诊进行新辅助化疗的患者为研究对象,进行半结构化深度访谈。采用Colaizzi 7步分析法分析资料。结果:乳腺癌新辅助化疗患者运动促进因素归纳为感知运动锻炼的益处和疾病因素引发的健康危机意识2个主题;阻碍因素提炼为自我效能低、化疗后状态不佳导致的身体活动受限以及自我形象紊乱3个主题。结论:乳腺癌新辅助化疗患者运动锻炼受多种因素影响,医护人员应采取针对性措施,提高患者运动参与度及疾病症状管理能力,以提高其化疗期运动依从性。

Abstract: Objective: To study the barriers and facilitators to exercise in patients with breast cancer undergoing neoadjuvanting chemotherapy, so as to provide a reference for formulating exercise intervention plans and constructing scientific exercise intervention programs. Methods: Phenomenological research method was applied to conduct semi-structured in-depth interviews with the first neoadjuvant chemotherapy patients in the breast surgery department of a grade A tertiary cancer hospital from August 1, 2023 to October 31, 2023, and the data were analyzed by Colaizzi 7-step analysis method. Results: The exercise facilitators of breast cancer patients with neoadjuvant chemotherapy could be summarized into two themes, including perception of the benefits of exercise and health crisis consciousness caused by disease factors;The barriers can be distilled into three themes, including low self-efficacy, limited physical activity due to a poor status after chemotherapy, and self-image disorder. Conclusion: Exercise in patients with neoadjuvant chemotherapy for breast cancer is affected by many factors. Medical staff should take targeted measures to improve patients’ exercise participation and disease symptom management ability, so as to improve their exercise compliance during chemotherapy.
