摘要:  

目的:探讨基于信息-动机-行为技巧模型的护理干预对喉癌患者全喉切除术后食管发音训练的影响。方法:随机选取喉癌行全喉切除术的112例患者为研究对象,随机分为干预组和对照组各56例。对照组予以常规护理,干预组在常规护理的基础上予以信息-动机-行为技巧模型的康复护理,比较两组患者术后食管发音言语康复的疗效、心理状态及生活质量的情况。结果:干预组成功食管发音50例(89%),对照组成功食管发音38例(68%),两组比较差异有统计学意义(P<0.01);干预前两组患者的心理状态评分和生活质量评分比较差异无统计学意义(P>0.05);干预后,干预组焦虑、抑郁得分明显低于对照组(P<0.01),干预组生活质量明显高于对照组(P<0.05)。结论:信息-动机-行为技巧模型的护理干预可以提高喉癌患者全喉切除术后食管发音的疗效,促进言语康复,降低患者的焦虑、抑郁水平,提高患者的生活质量。

Abstract:  

Objective: To explore the effect of nursing intervention based on information-motivation-behavioral skills model (IMB) on esophageal speech rehabilitation after total laryngectomy in patients with laryngeal cancer.Methods: A total of 112 patients who underwent total laryngectomy for laryngeal cancer were randomly divided into intervention group (56 cases) and control group (56 cases). The control group received routine care, and the intervention group provided rehabilitation nursing based on the information-motivation-behavioral skills model in addition to routine care. the efficacy of esophageal speech rehabilitation, the patient’s psychological status and the quality of life were compared between the two groups. Results: In the intervention group, 50 patients(89%) had successful esophageal pronunciation, and that was 38 (68%) in the control group, there were clinically significant differences between the two groups (P<0.01). There were no significant differences in psychological status scores and quality of life scores between the two groups before intervention (P>0.05). After the intervention, the SAS and SDS scores of the intervention group were significantly lower than the control group(P<0.01).The quality of life of the intervention group was significantly higher than that of the control group (P<0.05). Conclusion: The intervention focused on the information-motivation-behavioral skills model can improve the efficacy of esophageal speech training after total laryngectomy in patients with laryngeal cancer, promote the speech rehabilitation, reduce the anxiety and depression of patients, and improve the quality of life of patients.
