A longitudinal study on the correlations between symptom clusters and quality of life in patients with lymphoma during chemotherapy

Objective: To investigate symptom clusters and quality of life in lymphoma patients at different time points during chemotherapy, and to explore the dynamic changes in the correlation between symptom clusters and quality of life. Methods: A total of 159 patients with lymphoma who received initial chemotherapy were selected conveniently. The symptoms and quality of life of the patients were investigated before chemotherapy (T1), after the third cycle (T2) and after the sixth cycle (T3). Results: Before chemotherapy, patients had gastrointestinal symptom clusters, psychological symptom clusters and lymphoma B symptom clusters. After the third and sixth cycles of chemotherapy, lymphoma B symptom clusters disappeared, and chemotherapy-related symptom clusters increased. Among quality of life, the physical function, role function and cognitive function of patients in 3 cycles were compared, and the differences were significantly different ( P<0.01). Symptom groups were negatively correlated in each dimension of quality of life before chemotherapy and after the sixth cycle of chemotherapy ( P<0.05). Chemotherapy-related symptom group and psychological symptom group were negatively correlated in all dimensions of quality of life after the third cycle of chemotherapy ( P<0.05). Conclusion: There are multiple stable symptom clusters in lymphoma patients during chemotherapy, which are negatively correlated with quality of life. Clinicians should manage the symptoms of lymphoma patients during chemotherapy, reduce the burden of symptoms and improve the quality of life.

Qualitative study on the real experience of transitional period in survivors of diffuse large B-cell lymphoma

Objective: To explore the real experience of transitional period in survivors of diffuse large B-cell lymphoma, so as to provide reference for further improving the quality of life of survivors of diffuse large B-cell lymphoma. Methods: By purposive sampling method, a semi-structured interview was conducted in 14 patients with diffuse large B-cell lymphoma who had completed treatment and had follow-up visits from August to December 2021. The data were analyzed using the Colaizzi 7-step analysis method. Results: The real experience of survivors of diffuse large B-cell lymphoma during transitional period was summarized into three themes such as psychological experience themes including increased sense of life, high levels of fear of recurrence, and decreased individual confidence; physiological experience themes including fatigue, poor recovery of bone marrow function, and long-term symptom distress; social experience themes including striving to live, returning to normalcy, and seeking professional support and assistance. Conclusion: Survivors of diffuse large B-cell lymphoma during transitional period have multiple physiological, psychological, and social experiences. Medical institutions should strengthen attention to survivors and provide active and effective measures to help survivors of diffuse large B-cell lymphoma transition smoothly to survival.

Study of quality of life and its influencing factors in cancer patients receiving immune checkpoint inhibitor therapy

Objective: To investigate the status of quality of life and its influencing factors in cancer patients receiving immune checkpoint inhibitors(ICIs) therapy, and to provide basis for improving the quality of life of the patients. Methods: A total of 216 patients receiving ICIs therapy from September 2021 to February 2022 were included,the general information questionnaire, ECOG score,European organization for research and treatment of cancer quality of life questionnaire-core 30 and the symptom assessment scale were used in the investigation. Results: The lowest general health score of patients treated with ICIs therapy was 66.67(50.00, 83.33). The multiple linear regression analysis showed that treatment method, symptom burden and ECOG score were the influencing factors of body function and role function (Adj R 2 =0.485, 0.383). Meanwhile, age, gender, symptom burden and ECOG score were the influencing factors of emotional function (Adj R 2= 0.350), and age, symptom burden and ECOG score were the influencing factors of cognitive function(Adj R 2 =0.250). While, symptom burden and ECOG score were the influencing factors of social function and general health (Adj R 2 =0.201, 0.314). Conclusion: The general health status of patients treated with ICIs therapy is not well. We should pay attention to the patients who are young , female, receiving combined treatment, with high ECOG score and severe symptom burden, and implement effective nursing strategies to improve the quality of life in cancer patients.

Analysis of cognitive function status and influencing factors in elderly patients with lymphoma undergoing chemotherapy

Objective: To investigate the cognitive function of elderly lymphoma patients receiving chemotherapy and analyze its influencing factors, so as to provide scientific basis for improving the cognitive function. Methods: A total of 121 hospitalized elderly lymphoma patients with chemotherapy were investigated using self-designed general scale and the Montreal cognitive assessment scale (MocA). Results: The incidence of cognitive impairment was 81.0%, the score of cognitive function was 23.17±3.40. Multiple linear regression analysis showed that gender, urban and rural, educational level, chemotherapy regimen and self-management level were the relevant factors of cognitive function in elderly lymphoma chemotherapy patients, which accounted for 50.9% of the total variation. Conclusion: Because of the high incidence of cognitive impairment multiple influencing factors, clinical medical staff should strengthen the cognitive training of elderly patients for improve the quality of life of these elderly patients.
