Status and influencing factors of disease acceptance in patients with esophageal cancer

Objective: To investigate the status of disease acceptance, psychological resilience and social support in patients with esophageal cancer, and to analyze the influencing factors of disease acceptance in patients with esophageal cancer. Methods: Convenience sampling method was used to select 230 patients with esophageal cancer who were treated in a Class A tertiary hospital in Tianjin as the research objects. A questionnaire survey was conducted using general information questionnaire, disease acceptance scale, psychological resilience scale and social support scale. Results: The total score of disease acceptance in patients with esophageal cancer was 18.96 ±3.38, which was generally at a low level; disease acceptance was positively correlated with resilience, social support and all dimensions( r=0.290~0.488, P<0.05); multiple stepwise regression analysis showed that resilience, social support, place of residence, presence or absence of chemotherapy history, and education level had significantly effect on disease acceptance, explaining 39.3% of the total variance. Conclusion: The level of disease acceptance, psychological resilience and social support in patients with esophageal cancer needs to be improved. Psychological resilience, social support, residence, chemotherapy history, and education level are all influencing factors of disease acceptance. Medical staff should guide patients according to individual conditions, improve patients′ awareness of their own diseases, and pay attention to patients′ disease acceptance, so as to carry out targeted interventions to improve patients′ disease acceptance and better face the disease.

Analysis of the current situation and influencing factors of nurses' experience and support as the second victim of nursing adverse events

Objective: To understand the current situation of nurses’ experience and support as the second victim of adverse nursing events, and analyze the influencing factors. Methods: A total of 421 nurses who had experienced adverse nursing events in a hospital in Tianjin were included by convenience sampling method from January to March 2022. General data questionnaire, second victim experience and support scale and mental toughness scale were used for the survey, and the influencing factors were analyzed by multiple linear regression. Results: The total score of experience and support scale for nurses as the second victim of adverse nursing events was 91.74±12.66, and the total score of mental toughness scale for nurses who had experienced adverse nursing events was 90.12±17.58. There was no correlation between the second victim experience and the total score of support, the second victim total score of pain and mental resilience, and the scores of all dimensions( P>0.05). Organizational support was positively correlated with total score of mental toughness and self-strengthening ( P<0.05), while non-job-related support was positively correlated with total score of mental toughness, tenacity, optimism and self-strengthening( P<0.05). Multiple linear regression analysis showed that educational level, professional rank and understanding of the concept of "second victim" were the influencing factors of the pain of nurses as the second victim of adverse nursing events ( P<0.05). The number of night shift per month and the total score of mental toughness were influencing factors of organizational support of nurses(P<0.05). Conclusion: The experience and support level of nurses as the second victim of adverse nursing events need to be improved. Adverse nursing events have a negative impact on the physical and mental well-being of nurses as the second victim, and the available support is short. It is suggested that nursing managers should adopt targeted methods to guide and help nurses to alleviate the negative effects of adverse events on nurses’ physical and mental health according to the characteristics of nurses, and improve the experience and support level of nurses as the second victim of adverse nursing events by improving the mental toughness level.

Application and effect observation of competency model of clinical nursing teacher in training for clinical nursing teachers

Objective: To evaluate the application effect of clinical nursing teacher competency model in clinical nursing teacher training. Methods: 26 clinical nursing teachers who were recruited through competition in 2017 were assigned to the control group, while 25 clinical nursing teachers who competed in 2018 were included in the observation group.The control group adopted the traditional training method, and the observational group adopted the training scheme based on the competency model of clinical nursing teachers.The scores of theoretical knowledge, year-end comprehensive assessment and clinical nursing teachers' competence evaluation scale were compared between the two groups. Results: The observation group was superior to the control group in theory and year-end comprehensive assessment( P<0.01), and the observational group was superior to the control group in teaching knowledge and skills, self-concept, motivation and traitsin clinical nursing teachers' competence questionnaire( P<0.05). There was no significant difference in knowledge and skill dimensions( P=0.22). Conclusion: The competency training program for clinical nursing teachers based on the competency model of clinical nursing teachers is helpful to promote the role change, role adaptation and teaching skills of new clinical nursing teachers, and can be used in the training of clinical nursing teachers.

Study on the current status and influencing factors of meaning in life in patients with esophageal cancer

Objective: To investigate the status of meaning in life and self-esteem in esophageal cancer patients, to explore the correlation between them, and to understand the influencing factors of meaning in life in the patients. Methods: 212 patients with esophageal cancer in a tertiary Class A hospital in Tianjin were selected by the convenient sampling method. Questionnaires were surveyed using the general data questionnaire,Chinese version of meaning in life scale and self-esteem scale. Results: The total score of meaning in life of patients with esophageal cancer was 75.72±8.60, which was generally at a low level. The score of self-esteem was 25.05±3.23, which was at a low level. The scores of meaning in life and each dimension were positively correlated with the score of self-esteem( r=0.435~0.617, P<0.01).Multiple stepwise regression analysis showed that self-esteem, education, and marital status affected the meaning in life( P<0.01). Conclusion: Patients with esophageal cancer need to improve their sense of meaning in life and self-esteem. Medical staff should guide patients to adopt a positive attitude towards life, communicate effectively with patients, and provide targeted individualized interventions and psychological guidance to improve patients' self-esteem and sense of meaning in life.
