社会支持在未婚女性乳腺癌术后患者的应对方式和健康坚韧性间的中介作用

Study on mediating role of social support in coping style and health-related hardiness in unmarried women after breast cancer surgery

摘要: 目的:探讨社会支持在未婚女性乳腺癌术后患者健康坚韧性与应对方式间的中介作用。方法:采用目的取样选取天津市某三级甲等医院115例未婚女性乳腺癌术后患者为研究对象,采用应对方式问卷(SCSQ)、社会支持评定量表(SSRS)、健康坚韧性量表(RHHI-24)和自行设计的一般资料调查表进行问卷调查。结果:115例未婚女性乳腺癌术后患者社会支持总分为(36.65±7.38)分;积极应对得分(24.36±6.68)分;消极应对得分(11.39±4.15)分;健康坚韧性得分(76.83±21.46)分;患者健康坚韧性与社会支持总分、积极的应对方式呈正相关(P<0.05),与消极的应对方式呈负相关(P<0.05)。结构方程模型分析表明,社会支持正向预测健康坚韧性(β=0.65,P<0.001),消极应对负向预测健康坚韧性(β=-0.28,P<0.001),社会支持在积极应对与健康坚韧性之间具有完全中介作用。结论:未婚女性乳腺癌术后患者健康坚韧性水平中等偏低,临床工作中应关注患者的应对方式和社会支持状况,鼓励其多采取积极应对方式,并最大程度的为患者提供全方位的社会支持,以此提高未婚女性乳腺癌术后患者健康坚韧性,用更好的心理资源应对疾病,缓解负性情绪,提高生活质量。

Abstract: Objective: To explore the mediating role of social support in coping style and health-related hardiness in unmarried women after breast cancer surgery. Methods: The research objects were selected by target sampling, including 115 unmarried women after radical mastectomy for breast cancer from a tertiary A hospital in Tianjin. The simplified coping style questionnaire(SCSQ), social support rating scale(SSRS), revised health-related hardiness inventor(RHHI-24) questionnaire and self-designed general information questionnaire were used. Results: The total score of social support in 115 unmarried women after breast cancer surgery was 36.65±7.38; the score of positive coping was 24.36±6.68; the score of negative coping was 11.39±4.15; the score of health-related hardiness was 76.83±21.46. The score of health-related hardiness was significantly correlated with total score of social support and score of positive coping style(P<0.05), while was negatively correlated with the score of negative coping style(P<0.05). Structural equation model analysis showed that social support significantly positively predicted health-related hardiness(β=0.65,P<0.001), Negative coping significantly negatively predicted health-related hardiness(β=-0.28, P<0.001), and social support was a complete mediator between positive coping and health-related hardiness. Conclusion: Unmarried women had a moderately low level of health toughness after breast cancer surgery. In clinical work, we should pay attention to patients’ coping styles and social support status, encourage them to adopt more positive coping styles, provide all-round social support for patients to the greatest extent, in order to alleviate negative emotions, and improve the quality of life.
