肿瘤化疗患者行锁骨下静脉置管困难的心理体验及需求的质性研究

摘要: 目的:探讨肿瘤化疗患者行锁骨下静脉置管术困难的真实感受和需求,为制定个性化的护理措施提供依据。方法:采用目的抽样法,对15例恶性肿瘤化疗需置入锁骨下静脉导管且管路置入困难的患者进行半结构式访谈,应用Colaizzi 现象学7步分析法分析资料。结果:通过阅读、分析、反思、分类和提炼,归纳出肿瘤化疗患者行锁骨下静脉置管术困难的4 个主题: 置管前对锁骨下静脉置管导管留置的担心和顾虑、置管困难时心理体验复杂性和阶段性、置管后身体不适、强烈的健康知识需求。结论:肿瘤化疗患者行锁骨下静脉置管术困难的心理体验及需求呈现复杂性与阶段性特征,护士应根据患者置管前后的具体情况给予针对性护理,满足其个性化需求,以帮助患者顺利进行导管置入,改善患者置管体验。

Abstract: Objective: To explore the experience and needs of tumor patients with subclavian vein catheterization difficulty in chemotherapy, so as to provide basis for formulating personalized nursing measures. Methods: From June to October in 2020, 15 malignant tumor patients who needed subclavian vein catheter and had difficulties in placing catheter were interviewed by semi-structured interview. The data was analyzed by Colaizzi phenomenological 7-step analysis. Results: Through reading, analyzing, reflecting, classifying and refining, four themes were summarized: worries and concerns before catheterization, complex psychological experience when difficulties happened during catheterization, uncomfortable physical experience after catheterization, strong healthy information demands. Conclusion: The psychological experience and needs of tumor patients with subclavian vein catheterization difficulty in chemotherapy are complex and periodic changing. Nurses should give targeted nursing measures according to the specific situation of patients before and after catheterization to meet their individualized needs, so as to place catheters smoothly and improve patients’ experience during catheterization.
