脊柱外科术后患者便秘预防和管理的最佳证据总结

摘要: 目的:遴选、评价和总结脊柱外科术后患者便秘预防和管理的相关证据。 方法:针对脊柱外科术后患者便秘的管理提出问题,系统检索UpToDate、PubMed、国际脊髓损伤数据库、万方等中英文数据库和相关指南网的所有证据,包括临床决策、指南、证据总结和专家共识等。由2名经过循证培训的研究员独立筛选和评价相关文献,形成最佳证据总结。检索时间为建库至2020年9月。结果:共纳入14篇文献,从风险评估、诊断与筛查、预防和管理策略、评价指标、教育培训5个方面汇总了47条最佳证据。 结论:脊柱外科护士可结合研究汇总的证据完善和改进便秘管理制度,降低术后患者便秘发生率。在证据应用过程中需结合实际选择适合的最佳证据,以提高医护患各方满意度。

Abstract: Objective: To select, evaluate and summarize the best evidence for the prevention and management of constipation after spinal surgery. Methods: For the management of constipation in patients after spinal surgery, all the evidences, including clinical decision, guideline, evidence summary and expert consensus, were systematically searched in Up To Date, PubMed, international spinal cord society(ISCoS), Wanfang and other Chinese and English databases and related guideline networks. Two researchers independently evaluated literature quality and extracted evidence. The retrieval period was from the inception of databases to September, 2020. Results: A total of 14 literatures were included, and 47 best evidences were summarized from 5 aspects: risk assessment, diagnosis and screening, prevention and management strategies, evaluation indicators, and patient education. Conclusion: Nurses in spinal surgery can improve constipation management strategies and reduce the incidence of constipation by combining the evidence summarized in this study. Evidence users should select best evidence with consideration of clinical settings so as to improve the satisfaction of doctors and patients.
