乳腺癌相关性淋巴水肿患者治疗决策辅助需求的混合研究

A mixed study on the needs of treatment decision support in patients with breast cancer associated lymphedema

摘要: 目的:明确乳腺癌相关性淋巴水肿患者治疗决策困境的现状,以期为医护人员实施决策辅助提供参考。方法:采用量性及质性的混合研究方法。 量性研究采用目的性抽样法,选取 2021 年 5 月至 2022 年 5 月在天津市某三级甲等肿瘤专科医院就诊的乳腺癌相关性淋巴水肿患者 190 例作为研究对象进行问卷调查;质性研究通过目的性抽样和最大差异抽样,选取问卷调查中存在决策困境的患者进行质性访谈。 结果:回收有效调查问卷 187 份( 98.42% ),决策困境得分为 26.56 ( 16.06 , 38.28 )分,其中 45 例( 24.1% )患者得分 > 37.5 分,存在决策拖延, 102 例( 54.5% )患者得分 ≥25 分存在决策困境。 质性研究共提炼出 4 个主题,分别为乳腺癌相关性淋巴水肿知识及治疗认知不足;专业化决策信息不足;负性情绪的影响;支持系统不足。 结论:乳腺癌相关性淋巴水肿患者普遍存在治疗决策困境,提示医护人员应重视患者的健康教育、心理支持等,并给予相应干预措施。

Abstract: Objective: To determine the current status of treatment decision-making dilemma in patients with breast cancer- related lymphedema, so as to provide reference for the implementation of decision support for medical staffs. Methods: Quantitative and qualitative mixed research methods were adopted. In the quantitative study, purposive sampling method was used and 190 patients with breast cancer-related lymphedema were included from May 2021 to May 2022 in a Class A tertiary cancer hospital in Tianjin. Through purposive sampling and maximum difference sampling, patients with decision- making dilemma in the questionnaire survey were selected for qualitative interview. Results: A total of 187 valid questionnaires(98.42%) were collected, and the score of decision-making dilemma was 26.56(16.06, 38.28 ) points. Among them, 45 patients(24.1%) had a score of more than 37.5 points, indicating decision-making delay, and 102 patients(54.5%) had a score of more than 25 points, indicating decision-making dilemma. Qualitative research extracted four themes, which were insufficient knowledge of breast cancer related lymphedema and treatment, lack of professional decision-making information, the impact of negative emotions, and lack of support system. Conclusion: Patients with breast cancer-related lymphedema generally have treatment decision-making difficulties. Medical staffs should pay attention to health education and psychological support of patients, and give appropriate interventions.
