Effects of passive smoking during pregnancy on patients with gestational diabetes mellitus

孕期被动吸烟对妊娠糖尿病患者影响的研究

Abstract: Objective: To observe the effect of passive smoking during pregnancy on patients with gestational diabetes mellitus. Methods: The data of 177 patients who continued to participate in "one day outpatient service of gestational diabetes mellitus" was collected. According to whether passive smoking during pregnancy was divided into passive smoking group and non-passive smoking group, two groups of patients received the same " one day clinic for gestational diabetes " interventions. The fasting blood-glucose(FPG),2 hours postprandial blood glucose(2hPG), glycated hemoglobin (HbAlc), maternal outcomes and infant condition were analyzed. Results: The passive smoking rate during pregnancy among women with gestational diabetes mellitus was 45.20%. There was no significant difference in FPG、2hPG、HbAlc in the two groups before interventions (P>0.05), but there was difference after 2 months of interventions(P<0.05). There was no significant difference in delivery weeks, polyhydramnios, oligohydramnios, postpartum hemorrhage and preeclampsia between two groups (P>0.05), and there was statistical significance difference in delivery route and pregnancy-induced hypertension(P<0.05),There was no significant difference in rate of neonatal gender, length, 5-minute Apger score, hypoglycemia, malformation, hypocalcemia and asphyxia between two groups(P>0.05),and there was a statistically significant difference in neonatal body mass and rate of macrosomia (P<0.05). Conclusion: Passive Smoking during pregnancy is one of the risk factors affecting blood glucose control in patients with gestational diabetes mellitus, smoke environmental exposure is not conducive to disease control, and it is easy to lead to adverse maternal and child outcomes.

摘要: 目的: 观察孕期被动吸烟对妊娠糖尿病(GDM)患者的影响。方法: 收集持续参加“妊娠糖尿病一日门诊”177例患者的资料,按照是否被动吸烟分为被动吸烟组和不被动吸烟组。两组患者均接受“妊娠糖尿病一日门诊”干预,对比两组患者空腹血糖(FPG)、餐后2小时血糖(2hPG)、糖化血红蛋白(HbAlc)及分娩结局、新生儿情况。结果: GDM患者孕期被动吸烟率达45.20%。干预前FPG、2h PG、HbAlc差异无统计学意义(P>0.05);干预2个月后FPG、2hPG、HbAlc差异有统计学意义(P<0.05);分娩孕周、羊水过多、羊水过少、产后出血、先兆子痫的发生率差异无统计学意义(P>0.05);分娩方式、妊娠高血压发生率差异有统计学意义(P<0.05);新生儿性别、身长、5min-Apger得分、低血糖、畸形、低血钙、窒息的发生率差异无统计学意义(P>0.05);新生儿体质量、巨大儿发生率差异有统计学意义(P<0.05)。结论: 孕期被动吸烟是影响GDM患者血糖控制的危险因素之一,烟雾环境暴露不利于疾病病情控制,易导致不利的母婴结局。
