The application of precise positioning vibration expectoration in airway clearance of patients with acute exacerbation of chronic obstructive pulmonary disease

Abstract: Objective: To explore the effect of vibration expectoration guided by electrical impedance tomography (EIT) on airway clearance in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD). Methods: A total of 80 AECOPD patients admitted to the ICU of a grade A tertiary hospital in Tianjin were included and randomly divided into an observation group (n=40) and a control group (n=40). The control group received routine care, while the observation group received EIT-guided vibration expectoration in addition. Sputum drainage volume, oxygenation indices, serum hs-CRP, clinical pulmonary infection score (CPIS), and regional ventilation distribution (ROI 1~ROI 4) were compared between the two groups. Results: The observation group showed significantly higher sputum drainage volume, oxygenation index, and ROI 1~ROI 4 values, and significantly lower arterial partial pressure of carbon dioxide, CPIS, and hs-CRP than the control group (P<0.05). Conclusion: EIT-guided vibration expectoration can enhance sputum drainage, promote airway clearance, and facilitate pulmonary rehabilitation in AECOPD patients.

摘要: 目的:探讨电阻抗断层成像(Electrical Impedance Tomography,EIT)技术精准定位振动排痰在慢性阻塞性肺疾病急性加重(Acute Exacerbation of Chronic Obstructive Pulmonary Disease,AECOPD)患者气道廓清中的效果。方法:选取天津市某三级甲等医院ICU收治的 80 例 AECOPD 患者为研究对象,使用随机数字表将其分为观察组和对照组各 40 例。对照组进行常规护理,观察组在对照组基础上应用 EIT 技术进行精准定位振动排痰。比较两组患者痰液引流量、氧合指标、血清超敏 C 反应蛋白、临床肺部感染评分及肺部感兴趣区域(Region of Interest,ROI)1~ROI 4 值。结果:观察组痰液引流量、氧合指数以及 ROI 1~ROI 4 值高于对照组,动脉血二氧化碳分压、临床肺部感染评分、超敏 C 反应蛋白低于对照组,差异有统计学意义(P<0.05)。结论:EIT 技术精准定位振动排痰能够改善 AECOPD 患者的痰液引流的效果,促进患者气道廓清,有利于患者的肺康复。
