Evidence-based practice of prevention of medical device-related pressure injury in critically ill patients based on Knowledge to Action Cycle

知识转化理论在预防危重患者医疗器械相关性压力性损伤中的应用

Abstract:

Objective: To explore the effect of evidence-based practice on prevention of medical device related pressure injury (MDRPI) in critically ill patients based on the Knowledge to Action Cycle. Methods: According to the standard procedure of evidence-based practice in the Knowledge to Action Cycle, the evidence-based practice for the prevention of MDRPI in critically ill patients was formed and applied to clinical effect evaluation. A non-synchronous control trial design was used. Critically ill patients with MDRPI risk admitted from July to September 2019 were included in the control group. Critically ill patients admitted to the same clinical situation from July to September 2020 were included in the intervention group. The control group received routine care and the intervention group received evidence-based practice of prevention on the basis of routine care. The incidence of MDRPI between the intervention group and the control group was compared and the feasibility and effectiveness of the prevention evidence-based practice program in specific clinical situations was evaluated. Results:There was no significant difference in baseline data between the two groups (P>0.05). The incidence of MDRPI in the intervention group was lower than that in the control group, there was significant statistical difference (P<0.05).Conclusion:Evidence -based practice of MDRPI prevention among critically ill patients was carried out based on the Knowledge to Action Cycle Theory, and a scientific, feasible and evidence-based practice for MDRPI prevention was formed. It reduced the incidence of MDRPI, and improved the quality of skin management among critically ill patients.

摘要: 目的:探究基于知识转化理论的危重患者医疗器械相关性压力性损伤预防实践的应用效果。方法:依据知识转化理论中循证实践 的标准程序,最终形成危重患者医疗器械相关性压力性损伤预防循证实践方案,并将其应用于临床进行效果评价。 采用非同期对照试 验设计,将 2019 年 7 月至 9 月收治于 ICU 的具有医疗器械相关性压力性损伤发生风险的危重患者纳入对照组,将 2020 年 7 月至 9 月 在相同临床情境收治的危重患者纳入干预组,对照组采用 ICU 常规护理,干预组在常规护理基础上执行循证实践方案。 比较两组患者 在 ICU 期间医疗器械相关性压力性损伤的发生率。 结果:两组患者基线资料比较差异无统计学意义(P>0.05);干预组医疗器械相关性 压力性损伤发生率低于对照组,差异有统计学意义(P<0.05)。结论:基于知识转化理论进行危重患者医疗器械相关性压力性损伤预防实 践,形成了具备科学性及可行性的预防实践方案,可降低医疗器械相关性压力性损伤发生率,提高危重患者皮肤管理质量。
