Objective: To explore the application of shared care management platform in patients with gestational diabetes mellitus and the effect of health management intervention. Methods: A total of 90 patients admitted to the department of respiratory medicine of a class A tertiary hospital in Zhengzhou from February 2020 to January 2021 were included and were divided into the observation group and the control group according to the envelope method, with 45 patients in each group. The control group adopted routine follow-up management, while the observation group adopted shared care management platform. The intervention effect were evaluated by comparing fasting blood glucose（FBG）, 2-h postprandial blood glucose（2 hPG）, glycosylated hemoglobin. The knowledge of disease was assessed by diabetes knowledge questionnaire，and self-management ability was assessed by Self-management Ability Questionnaire for Gestational Diabetes Mellitus Patients(SMQGDM). Results:After intervention, the observation group showed lower mean levels of FBG, 2 hPG and glycosylated hemoglobin, and higher mean scores of related knowledge, and higher mean total score of SMQGDM compared with the control group（P<0.05）.Conclusion: The application of shared care management platform in patients with gestational diabetes is conductive to improving the related knowledge, glycemic control and self-management ability. 

目的：探讨共同照护管理平台在妊娠期糖尿病患者中的应用及健康管理干预效果。 方法：选择 2020 年 2 月至 2021 年 1 月在郑 州市某三级甲等医院门诊及住院后回归社区的 90 例患者作为研究对象，使用信封法分为观察组和对照组。 对照组采用科室常规的 健康指导及随访管理，观察组采用共同照护管理平台进行管理。 采用空腹血糖、餐后 2 h 血糖、糖化血红蛋白比较两组的血糖控制状 况；采用糖尿病知识问卷比较两组的疾病相关知识掌握情况；采用糖尿病自我管理能力问卷比较两组的自我管理能力。结果：干预后，观察组血糖控制水平优于对照组， 疾病相关知识掌握情况优于对照组， 自我管理能力得分高于对照组， 差异均有统计学意义 （P< 0.05）。 结论：应用共同照护管理平台对妊娠期糖尿病患者进行健康管理，有利于提高其知识水平，改善患者的自我管理能力，达到控 制血糖的目的。
