Abstract

Abstract: Objective：To investigate how to reengineer the process of safety verification of surgical/invasive procedure and extend of the application according to the JCI standards. Methods： The International Patient Safety Goals (IPSG.4) was conducted to reengineer the process of the surgical/invasive procedure safety verification. Trainings were organized for medical staffs to familiarize with the process and abide by the standards. The Plan-Do-Check-Action (PDCA) management tools practically used for quantitative assessment，supervision，and inspection. Results： The implementation rate was increased from 52.1% to 99.5% and the qualified rate from 44.9% to 97.6%（P<0.001），through three rounds of PDCA cycle. The statistical differences were significantly indicated and achieved of the aims of the IPSG.4. Conclusion： JCI standards combined with the application of management tools were revealed of the improvement of the qualification rate of the surgical/invasive procedure safety verification, and preclude of the errors of the correct-patient surgery， site，and procedure.

摘要：

目的：探讨如何按照JCI标准再造手术/有创操作安全核查流程，并推广应用。方法：遵照IPSG.4修订手术/有创操作安全核查制度，再造流程；对医护人员专项培训，掌握制度、熟悉流程、恪守标准、规范执行；应用管理工具PDCA，量化考核，监督检查。结果：通过三轮PDCA循环，将手术/有创操作安全核查的执行率由创建之前的52.1%提高至99.5%、合格率由44.9%提高至97.6%，前后比较差异均有统计学意义（P<0.001），实现了JCI标准中IPSG.4的目标。结论：以JCI标准为准则，结合管理工具的应用，有效提升了手术/有创操作安全核查的合格率，杜绝了手术时患者、部位及操作发生错误。
