Experience of social isolation in patients with bladder cancer: a qualitative study

Objective: To describe and analyze the experience of social isolation in patients with bladder cancer, and to provide evidence for developing intervention strategies for social isolation in these patients. Methods: A phenomenological research approach was used. Fourteen patients with bladder cancer, who visited a grade Ⅲ and first-class hospital in Tianjin from August 2023 to November 2023, were selected as the study subjects. Face-to-face semi-structured interviews were conducted, and Colaizzi′s seven-step method was used to synthesize and extract themes. Results: Two main themes were identified, including subjective social isolation and objective social isolation. Seven sub-themes emerged, including feelings of loneliness, a sense of burden, negative perceptions of threats, feelings of life disparity or numbness, changes in interests and hobbies, decline in social engagement, and the formation of a family cocoon. Conclusion: The experience of social isolation in bladder cancer patients mainly includes subjective and objective aspects of social isolation. Healthcare providers should develop a social network for bladder cancer patients, guided by family involvement and personal interests, by focusing on individualized care, family support, and reallocation of social information resources.

The status and influencing factors of attitude to dietary therapy in hemodialysis patients

Objective: To investigate the status of attitude to dietary therapy in hemodialysis(HD) patients and analyze its influencing factors. Methods: A total of 285 participants who underwent HD treatment in the outpatient department of kidney disease blood purification center of a Class A tertiary hospital in Tianjin were recruited by convenience sampling method from March 2019 to April 2019. The general information questionnaire, the Attitude Scale for the Dietary Therapy of Hemodialysis Patients (ASDTH), the Positive and Negative Affect Schedule (PANAS), the Brief Illness Perception Questionnaire(BIPQ), the end stage renal disease dietary knowledge questionnaire, and the Social Support Rating Scale (SSRS) were used to the investigation. Results: The total score of attitude to dietary therapy in HD patients was 40.0 (37.0, 43.0). There were 150 patients(52.6%) with moderate dietary therapy attitude. Multiple stepwise regression analysis showed that social support, dietary knowledge, illness perception, drinking, male, and preparing food by oneself were influencing factors of attitude to dietary therapy in HD patients ( P<0.05). Conclusion: Attitude to dietary therapy in HD patients is at the above medium level. It suggests that scientific and effective personalized health education programs should be provided to different patients and their families in clinical working. Patients’ emotions should be given attention in time and patients who are male especially with drinking and in charge of food by themselves should be given more active communication, so as to change unhealthy lifestyle habits.

Study on health-related quality of life in urolithiasis patients based on EQ-5D utility score system

Objective: To investigate the level and influencing factors of health-related quality of life in urolithiasis patients.Methods: A total of 189 urolithiasis patients who received inpatient treatment in the department of urology in a Class A tertiary hospital in Tianjin were included by convenience sampling method. The five-dimension European Quality of Life (EQ -5D) scale was used for investigation. Univariate analysis and multivariate generalized linear model were used to analyze the influencing factors of patients’ quality of life. Results: Patients had the most problems in the dimensions of pain/discomfort, anxiety and depression, with a health utility value of 0.942(0.893, 1.000). Age, gender, presence of ureteral stents, pain, and chronic disease were influencing factors of health-related quality of life in urolithiasis patients( P<0.05).Conclusion: The health-related quality of life in urolithiasis patients needs to be improved. It is necessary to understand the influencing factors and formulate a scientific and reasonable intervention program, so as to give targeted measures.

Investigation on the status and influencing factors of depression in patients with coronavirus disease 2019

Objective: To explore the correlation between depression, social support and coping style in patients with coronavirus disease 2019 in mobile cabin hospitals. Methods: Self-rating depression scale, perceived social support scale and simplified coping style questionnaire were used to investigate patients admitted to mobile cabin hospitals. Pearson correlation and hierarchical regression were used to analyze the relationship between depression, social support and coping style. Results: A total of 94 patients with coronavirus disease 2019 were included in the study. The average age of the patients was 46.81±11.34 years old, and the average stay time in the mobile cabin hospitals was 14.48±6.62 days. The overall depression score of the patients was 43.01±11.22 points. Depression was negatively correlated with social support and positive coping styles( P<0.01). Hierarchical regression results showed that social support and coping style had a predictive effect on the depression level in patients with coronavirus disease 2019( P<0.01). Conclusion: The depression level in patients with coronavirus disease 2019 in mobile cabin hospitals is relatively high. Social support and positive coping styles are negatively correlated with depression, which are predictive factors of depression in patients with coronavirus disease 2019.

Application effect of Omaha System in nursing care of female patients with stress urinary incontinence

Objective: To explore the application effect of Omaha System in female patients with stress urinary incontinence. Methods: A total of 84 cases of the patients with stress urinary incontinence were selected and were divided into control group(42 cases) and observation group(42 cases) randomly. Both groups were treated with tension-free vaginal tape-obturator(TVT-O) under epidural anesthesia. Patients in the control group received routine nursing care, and the observation group received nursing care based on Omaha System in addition to routine nursing. The early postoperative complications of the two groups were observed and the quality of life and anxiety of the patients were evaluated by incontinence quality of life questionnaire(I-QOL)and self-rating anxiety scale(SAS). Results: The incidence of postoperative complications in the observation group was significantly lower than that in the control group ( P<0.05); I-QOL score in the observation group was higher than that in control group , SAS score was significantly lower than that in control group( P<0.05). Conclusion: Nursing care based on Omaha System can reduce the incidence of postoperative complications in female patients with stress urinary incontinence underwent TVT-O and improve the quality of life of patients, meanwhile, reduce the degree of anxiety of patients effectively.

Research and analysis on the present situation and countermeasures of clinical practice of foreign students

Objective: To explore the deficiencies in clinical practice teaching for foreign students and find effective improvement strategies. Methods: By means of content analysis, 10 international students who were in our hospital for clinical practice were selected to conduct semi-structured interview, collect and summarize the interview data to extract key information, and analyze the underlying problems, so as to find the best solution. Results: According to the interviewed foreign students, there were some problems, such as weak clinical communication ability, few operational opportunities, and post competence should be improved. The teachers need to strengthen the alliance between hospitals and university, improve students' practical ability and improve their post competence. Conclusion: In the training system for foreign students, the number and proportion of practical classes should be increased, and they should have enough operation opportunities in clinical practice,including both simulation and actual operations. Therefore, the multi-linkage and mutual recognition training mode with policy as the upper guarantee, colleges as the main body, post competence as the starting point, and ability shaping as the goal, so as to promote the collaboration between hospital and school and improve the effect of foreign students training.
