Analysis of the current status and influencing factors of treatment decision-making dilemma in patients with pulmonary nodules

Objective: To investigate the current status of treatment decision-making dilemma in patients with pulmonary nodules and to analyze the influencing factors. Methods: A survey was conducted on 253 patients with pulmonary nodules using the general information questionnaire, Decision Conflict Scale (DCS), Hospital Anxiety and Depression Scale (HADS) and Perceived Social Support Scale (PSSS). Univariate and multiple linear regression analysis were used to analyze the influencing factors of treatment decision-making dilemma in patients with pulmonary nodules. Results: The mean DCS score for patients with pulmonary nodules was 30.03±8.28, with 187 patients (73.91%) scoring ≥25 points. Multiple linear regression analysis showed that cognition of the disease, anxiety score and social support level were influencing factors for decision-making dilemma. Conclusion: Treatment decision-making dilemma is prevalent among patients with pulmonary nodules. Clinicians and healthcare providers should pay attention to patients′ cognitive level of the disease and psychological characteristics, offering appropriate decision aids or psychological support to reduce decision difficulties.

The effects of imagination-reality exposure therapy on exercise fear in patients with chronic heart failure combined with hypertension

Objective: To explore the effects of imagination-reality exposure therapy on exercise fear in patients with chronic heart failure combined with hypertension. Methods: A total of 126 patients with chronic heart failure combined with hypertension who were hospitalized in the cardiology department were included from February 2020 to January 2022 by convenience sampling method. According to the random number table method, the patients were divided into the observation group and the control group with 63 cases in each group. The control group received routine nursing intervention, and the observation group received imagination-reality exposure therapy on the basis of routine nursing intervention for 12 weeks. The scores of Chinese version of the Tampa Scale for Kinesiophobia Heart (TSK-Heart-C), Self-efficacy for Exercise (SEE) and Connor-Davidson Resilience Scale(CD-RISC) were compared between the two groups. Results: During the study period, 4 cases dropped out of the study with 2 cases in each group. Finally, 61 cases in each group were included in the study. After intervention, the scores of exercise avoidance, perceived risk, self-dysfunction, exercise fear and total TSK-Heart-C in the observation group were significantly lower than those of the control group ( P<0.05);the SEE and CD-RISC scores of the observation group were significantly higher than those of the control group( P<0.05). Conclusion: Imagination-reality exposure therapy can improve the exercise self-efficacy and psychological resilience of patients with chronic heart failure combined with hypertension with exercise fear, and reduce exercise fear.

Application of health management model based on disease risk stratification in young and middle-aged patients with prehypertension

Objective: To explore the effect of health management model based on disease risk stratification on young and middle-aged patients with prehypertension. Methods: A total of 256 young and middle-aged patients with prehypertension were randomly divided into observation group and control group with 128 cases in each group. The control group received routine health education, while the observation group received health management model based on disease risk stratification. Results: After 6 months of intervention, the levels of overweight, obesity, excessive salt intake, insufficient exercise, unreasonable diet, and drinking in the observation group were lower than the control group( P<0.05). There was no significant difference between smoking and psychological problems( P>0.05). There was significant difference between unreasonable diet and drinking in the trend test (P<0.05).After 3 and 6 months intervention, the scores of self-health management behaviors in the observation group were significantly higher than those in the control group( P<0.05), and it tended to increase with time( P<0.001).The levels of SBP and DBP in the observation group were significantly lower than those in the control group, and it tended to decrease with time( P<0.001). Conclusion: The health management model based on disease risk stratification can effectively decrease the health risk factors for young and middle-aged patients with prehypertension and promote self-health management of the patients, so as to control blood pressure effectively.

The design and application of specialized restraints for fiberoptic bronchoscopy in children

Objective: To investigate the efficacy of a self-designed restraint for children undergoing alveolar lavage for lobar pneumonia with fiberoptic bronchoscopy. Methods: A total of 196 children who received alveolar lavage from January to June 2019 were selected as the control group, and 181 children who received alveolar lavage from July to December 2019 were selected as the observation group. The special restraint was used in the observation group for children undergoing fiberoptic bronchoscopy, while the control group used the traditional restriction band. The heart rate, oxygen saturation of the two groups of children before alveolar lavage were collected, and the changes of heart rate and oxygen saturation during alveolar lavage were compared. The time-consuming and labor cost of two groups of children undergoing alveolar lavage were compared. Results: There were no statistically significant differences in heart rate and oxygen saturation between the observation group and the control group before and during alveolar lavage( P>0.05). There were statistically significant differences in heart rate and oxygen saturation of the control group before and during alveolar lavage ( P<0.01). There were significant differences in heart rate and oxygen saturation between the two groups during alveolar lavage, and there were statistically significant differences between the two groups in time consuming and labor cost during alveolar lavage( P<0.01). Conclusion: The specialized restraint for children undergoing alveolar lavage for lobar pneumonia with fiberoptic bronchoscopy can stabilize the heart rate and oxygen saturation of children, effectively shorten the time of alveolar lavage, and decrease the labor cost of restraint.

Study on the effect of multidisciplinary collaborative nursing model on self-perceived burden and quality of life of patients with recurrence of primary liver cancer after operation

Objective: To explore the effect of multidisciplinary collaborative nursing model on self-perceived burden and quality of life of patients with recurrence of primary liver cancer after operation. Methods: 200 patients with postoperative recurrence of primary liver cancer were randomly divided into observation group and control group with 100 cases in each group. The control group was given routine nursing, and the observation group was given multidisciplinary collaborative nursing intervention on the basis of the control group. The scores of self-perceived burden scale and quality of life were compared between the two groups before and after nursing. Results: There was no significant difference in the scores of self-perceived burden scale, total score of quality of life and each dimension between the two groups before intervention( P>0.05). After the intervention, the scores of self-perceived burden scale, total score of quality of life and each dimension in the observation group were significantly higher than those in the control group ( P<0.05). Conclusion: Multidisciplinary collaborative nursing model can not only reduce the self-perceived burden of patients with recurrence of primary liver cancer, but also improve the quality of life of patients.

Effect of home care on self-management of patients with chronic heart failure based on timing theory

Objective: To explore the effect of home care on the self-management of patients with chronic heart failure (CHF) based on the theory of timing. Methods: 23 patients with first onset heart failure in cardiology from January to June 2018 in the observation group were given home care based on timing theory; 25 patients from January to December 2017 in control group were received routine care and follow-up. The level of self-management, quality of life and re-admission rate were recorded at 6 months after discharge. Results: The patients in observation group had higher level of self-management, lower scores of living quality, and lower re-admission rate than those of in the control group( P<0.05). Conclusion: Home care based on timing theory can effectively improve the self-management ability and quality of life, and reduce the rate of re-admission of CHF patients.
