Topic analysis of researches on "Internet+nursing service" in China based  on strategic coordinate map

Objective: To analyze the research theme and development trend of "Internet +nursing services" in order to provide reference for the high quality and sustainable development of "Internet+nursing services" in China. Methods: Taking "Internet+nursing service" as the theme, the literature published by CNKI from the inception of the database to December 2022 was retrieved. The bibliometric analysis software BICOMB and SPSS were used for metrological and cluster analysis of the included literatures, and the strategic coordinate map of the research topics was drawn to reveal the core themes and their development trends. Results: A total of 327 valid literatures were included, the number of published articles increased rapidly, and the distribution of journals was relatively concentrated. Nine research topics were identified by keyword clustering, among which the core and mature topics were "Internet+nursing service" nurse-patient experience, "Internet+nursing service" cognitive willingness needs of both nurse-patient and continued nursing of chronic diseases in the elderly. The core immature theme was "Internet+nursing service" quality. Theme to be developed included door -to -door service, the current situation and influencing factors of "Internet +nursing service", risk prevention and control of "Internet+nursing service" and nurse training. Conclusion: The "Internet+nursing service" in China is at the stage of rapid development and has attracted the attention of many researchers. Each research topic has different development stages and gradually develops to the connotation of "Internet +nursing service". Researchers should pay attention to the progress of hot topics and further promote the research of potential topics such as risk prevention and control and quality evaluation.

Investigation on postpartum outcome and health care of pregnant women with gestational diabetes mellitus

Objective: To investigate the status quo of postpartum outcome and health care needs of pregnant women with gestational diabetes mellitus (GDM) for 2 years after delivery, to provide reference for improving postpartum health care of them and reducing adverse outcomes. Methods: The convenience sampling method was used to investigate the status quo of postpartum outcomes and health care needs of GDM pregnant women who delivered in a third grade A hospital in Tianjin within 2 years. Results: About 13.6% of GDM pregnant women had postpartum body mass detention, 56.5% of them never monitored blood glucose postpartum, 4.2% of them had impaired glucose tolerance at different times after delivery, one GDM pregnant women had been diagnosed with type 2 diabetes within 2 years after delivery; 59.7% of them received postpartum care within 42 days after delivery, 34.6% of them never had postpartum care, 36.1% of them did not receive any relevant professional guidance.GDM pregnant women prefered to have obstetricians and gynecologists, midwives, and community health care doctors providing neonatal health care guidance, dietary guidance, exercise guidance, and health eduction for primary caregives through pregnancy school courses, brochures, and WeChat public platform, etc. Conclusion: At present, GDM pregnant women pay less attention to blood glucose changes and have poor awareness of self-monitoring. Postpartum health care is not systematic and standardized. In the future, screening and management should be further strengthened to reduce the occurrence of postpartum adverse outcomes of GDM.

Comparison of the efficacy of three frailty assessment tools on predicting postoperative complications in elderly patients undergoing abdominal surgery

Objective:To compare the efficacy of Frailty Phenotype(FP), Frail Scale(FS) and The Edmonton Frail Scale (EFS) on predicting the postoperative complications in elderly patients undergoing abdominal surgery, so as to provide reference for the selection of frailty assessment tools. Methods: A total of 184 elderly patients undergoing abdominal surgery aged ≥60 years old in a tertiary Class A hospital were included by convenience sampling method. The premorbid frailty status was assessed by FP, FS and EFS. The general information, the type of operation, the operation method and the postoperative complications were collected. Chi-square test was applied and the receiver operating characteristic (ROC) was used to evaluate the predictive efficacy of FP, FS and EFS. Results: The frailty detection rates of FP、FS and EFS in 184 elderly patients undergoing abdominal surgery were 22.3%,16.8%and 22.2%,resectively. There was no significant difference in the frailty detection rates of the three assessment tools( P>0.05). The area under the curve (AUC) in prediction of postoperative complications in elderly patients with abdominal elective surgery using FP was 0.798, which was significantly higher than that of FS(0.681) and EFS(0.670)( P<0.05). Conclusion: The frailty of elderly patients undergoing abdominal surgery were consistent with the three frailty assessment tools. Compared with FS and EFS, FP has higher efficacy on predicting the occurrence of postoperative complications in elderly patients undergoing abdominal surgery.

Reliability and validity of the Chinese version of postextubation dysphagia screening tool for patients with mechanical ventilation

Objective:To translate the English version of postextubation dysphagia screening tool(PEDS) into Chinese version and to test its reliability and validity in patients with mechanical ventilation. Methods: According to the Brislin translation model, the English version of postextubation dysphagia screening tool was translated into Chinese version, and then was back-translated and modified for cultural adaptation following the guidelines. Reliability test included Cronbach’s α coefficient and intra-class correlation coefficients. Validity test included Content Validity Index and exploratory factor analysis. The reliability and validity of the Chinese version of PEDS was tested in 100 patients with mechanical ventilation. As a golden standard for evaluation of dysphagia, flexible endoscopic examination of swallowing test was used to evaluate the sensitivity and specificity of PEDS. Results: The Cronbach’s α coefficient of the Chinese version of the scale was 0.904 and the intra-class correlation coefficients was 0.93. The Scale-Content Validity Index(S-CVI) was 0.96 and the Item-Content Validity Index(I-CVI) ranged from 0.84 to 1.00. The prevalence of postextubation dysphagia was 56% in 100 eligible patients.The sensitivity of the postextubation dysphagia screening tool was 82.1%and specificity was 70.5%. Conclusion: The Chinese version of PEDS has good reliability and validity, which can effectively evaluate the postextubation dysphagia risk in patients with mechanical ventilation.

Study on the correlation between mental health level and trait coping styles of patients with novel coronavirus pneumonia in Tianjin

Objective: To investigate the current status of mental health and trait coping styles of the patients with novel coronavirus pneumonia in Tianjin, and to explore the relationship between them, so as to provide scientific basis for clinical first-line nurses to carry out psychological intervention. Methods: 72 patients with novel coronavirus pneumonia treated in the tertiary hospital in Tianjin were surveyed by general data questionnaire, generalized anxiety disorder(GAD-7), patient health questionnaire(PHQ-9), and trait coping style questionnaire(TCSQ). Results: The GAD-7 score of patients with novel coronavirus pneumonia was 7.23±2.17, the anxiety incidence rate was 72.22%(52/72). The PHQ-9 score was 8.20±2.01, and the depression incidence rate was 70.83%(51/72). The total scores of GAD-7 and PHQ-9 in patients with novel coronavirus pneumonia were negatively correlated with the positive coping(PC) score( P<0.05), and positively correlated with the negative coping(NC) score( P<0.05). Conclusion: Patients with novel coronavirus pneumonia generally have different levels of mental health problems, which are closely related to the way coping styles of the disease. Medical staff should take targeted measures to maintain their mental health and promote disease rehabilitation while giving treatment to patients.

Analysis of the status quo and influencing factors of medical narrative ability of oncology specialist nurses

Objective: To investigate the status of medical narrative ability of oncology specialist nurses and analyze its influencing factors. Methods: 220 oncology specialist nurses by convenience sampling method were investigated by Chinese medical narrative ability scale and family care scale. Results: The total score of the medical narrative ability of 220 oncology specialist nurses was 158.64±17.13. There were statistically significant differences in the medical narrative ability scores of oncology specialist nurses with different educational backgrounds, working years, departments, marital status, and whether any of their relatives had cancer( P<0.05). Multiple linear regression analysis showed that department, marital status and family care level were the main factors affecting the medical narrative ability. Conclusion: The medical narrative ability of oncology specialist nurses is at the upper-middle level and needs to be further improved. Nursing managers should formulate targeted intervention measures based on the influencing factors of medical narrative ability to improve their medical narrative ability.

Study on fear of recurrence and its influencing factors in patients with primary breast cancer

Objective: To explore the current situation and influencing factors of relapse fear in patients with primary breast cancer, and to provide reference for nursing intervention in the future. Methods: 208 patients with primary breast cancer after surgery were selected by convenience sampling. They were surveyed by the questionnaires included general information questionnaire, Chinese big five personality questionnaire and concern about recurrence scale(CARS). Results: The total score of CARS was 80.63±18.513, and there were statistically significant differences in CARS scores among patients of different ages, residence, educational level, family per capita income, religious belief and TNM disease stage( P<0.05).Among the five personality characteristics, the agreeableness score was the highest, and the extraversion score was the lowest.Multiple linear regression analysis showed that age, residence, educational level and family per capita income of patients had a negative predictive effect, while neurotic personality had a positive predictive effect( P<0.05). Conclusion: There are different degrees of fear of recurrence in patients with primary breast cancer after surgery. Neuroticism is positively correlated with fear of recurrence, and agreeableness, extraversion, openness and conscientiousness are negatively correlated with fear of recurrence.It is suggested that medical staff analyze the source of fear according to different characteristics of patients, and study the correlation between fear of recurrence of cancer and personality traits, so as to provide basis for clinical development of more scientific nursing measures.

Establishment and application of standardized pressure ulcer management system

Objective: To establish a standardized nursing management system of pressure ulcer and evaluate its applying effects.  Methods: Since 2013 many measures were used such as establishing skin treatment group, analyzing problems of pressure ulcer management, establishing three level supervision system for managing pressure ulcers and system for assessing pressure ulcers , perfecting system of reporting, consulting and evaluating pressure ulcers , finally a set of standardized pressure ulcer management system is developed. Results: From 2013 to 2015, the cure / improvement rate of pressure ulcers out of hospital was 60.69%, 71.73% and 82.31%; the risk assessment accuracy, the correct rate of pressure ulcer prevention measures, pressure ulcer evaluation accuracy, accuracy of pressure ulcer nursing measures and patient satisfaction rate showed a rising trend year by year, and the difference had statistical significance (P<0.05). Conclusion: The standardized management system is more scientific, can improve the cure effect of pressure ulcers, ensure the implementation of nursing measures in place, and improve the quality of pressure ulcer care and management level.
