肿瘤科护士自我同情与情绪调节对职业倦怠的影响研究

The Influence of self-compassion and emotion regulation on occupational burnout among oncology nurses: a cross-sectional study

摘要: 目的:调查肿瘤科护士自我同情和情绪调节对职业倦怠的影响。方法:采用一般资料调查表、自我同情量表、情绪调节量表、 职业倦怠量表对 3 所三级甲等肿瘤专科医院的 259 名肿瘤科护士进行横断面调查。结果:259 名肿瘤科护士职业倦怠中情感衰竭维度得分为 25.00(16.00,32.00)分,76.9%的护士情感衰竭得分>26 分;去人性化维度得分为 11.00(6.00,15.00)分,54.1%的护士去人性化得分>9 分;个人成就感得分 32.00(28.00,37.00)分,56.8%的护士个人成就感得分<34 分。分层回归分析结果显示,自我同情(β=-0.378)、认知重评(β=-0.307)对情感衰竭有负向预测作用;月夜班数(β=0.937)、表达抑制(β=0.192)对去人性化有正向预测作用,自我同情(β=-0.147)、认知重评(β=-0.179)对去人性化有负向预测作用;工作时间(β=1.353)、自我同情(β=0.091)、认知重评 (β=0.573)对个人成就感有正向预测作用。结论:肿瘤科护士职业倦怠水平较高,自我同情、情绪调节的认知重评对职业倦怠各维度均有影响,情绪调节中的表达抑制对去人性化维度有影响。护理管理者可通过提升肿瘤科护士自我同情水平及情绪调节能力来 降低职业倦怠水平。

Abstract: Objective: To investigate the influence of self-compassion and emotion regulation on occupational burnout among oncology nurses. Methods: In this cross-sectional study, a total of 259 oncology nurses selected from three tertiary hospitals were evaluated by the general information questionnaire, Self-compassion Scale, Emotion Regulation Scale, and Maslach Burnout Inventory. Results: The median score of emotion exhaustion was 25.00 (16.00, 32.00), and 76.9% of the participants scored >26. The median score of depersonalization was 11.00 (6.00, 15.00), and 54.1% of the participants scored >9. The median score of personal accomplishment was 32.00 (28.00, 37.00), and 56.8% of the participants scored <34. Hierarchical multiple regression showed that self-compassion ( β=-0.378) and cognitive reappraisal ( β=-0.307) both had negative predictive effect on emotional exhaustion. Monthly night shift ( β=0.937) and expression inhibition ( β=0.192) both had a positive predictive effect on depersonalization dimension, while self-compassion ( β=-0.147) and cognitive reappraisal ( β=-0.179) had a negative predictive effect on depersonalization dimension. In terms of personal accomplishment, working hours ( β=1.353), self-compassion ( β=0.091) and cognitive reappraisal ( β=0.573) all had positive predictive effect on it. Conclusion: The level of job burnout of oncology nurses was at high level. The self-compassion and cognitive reappraisal of emotion regulation had influence on differnet dimensions of occupational burnout, and expression inhibition had influence on the depersonalization. With regard to nursing manager, it′ s suggested that the level of self-compassion and emotional regulation needed to be improved to reduce the level of occupational burnout.
