心理韧性在晚期妊娠初产妇社会支持与分娩恐惧间的中介效应

The mediating effect of mental resilience between social support and fear of childbirth among primiparas in late-pregnancy

摘要: 目的:探讨心理韧性在晚期妊娠初产妇社会支持与分娩恐惧间的中介效应。方法:采用便利抽样法,选取 2022 年 10 月至2023 年 10 月收治的 186 例晚期妊娠初产妇作为调查对象。采用一般资料调查表、中文版领悟社会支持量表、心理韧性量表、分娩恐惧量表对晚期妊娠初产妇进行问卷调查。采用 Pearson 相关分析法分析晚期妊娠初产妇社会支持、心理韧性与分娩恐惧的相关性;采用 AMOS 24.0 构建结构方程模型分析心理韧性在晚期妊娠初产妇社会支持与分娩恐惧间的中介效应,并应用 Bias-Corrected Bootstrap 法进行心理韧性中介效应的检验。结果:186 例晚期妊娠初产妇社会支持总分为(47.62±7.20)分,心理韧性总分为(23.36±6.10)分,分娩恐惧总分为(35.66±6.62)分。Pearson 相关分析显示,晚期妊娠初产妇社会支持与心理韧性呈正相关(=0.665,P<0.001),与分娩恐惧呈负相关(r=-0.478,P<0.001);心理韧性与分娩恐惧呈负相关(r=-0.510,P<0.001)。Bias-Corrected Bootstrap 法检验结果显示,心理韧性在晚期妊娠初产妇社会支持与分娩恐惧间具有部分中介效应(P<0.001),其中介效应值为-0.143,直接效应值为-0.308,总效应值为-0.451,中介效应占总效应的 31.71%。结论:心理韧性在晚期妊娠初产妇社会支持与分娩恐惧间具有中介效应,临床医护人员可通过提高心理韧性水平和社会支持来改善初产妇的分娩恐惧。

Abstract: Objective: To explore the mediating effect of mental resilience between social support and fear of childbirth among primiparas in late-pregnancy. Methods: A total of 186 primiparas in late-pregnancy from October 2022 to October 2023 were included by convenience sampling method. Then, the general data questionnaire, Chinese version of insight social support scale (PSSS), mental resilience scale (CD-RISC-10) and fear of childbirth scale (CAQ) were used to investigate social support, mental resilience and fear of childbirth of the primiparas in late-pregnancy. Pearson correlation was used to analyze the correlation between social support, mental resilience and fear of childbirth. AMOS 24.0 statistical software was used to construct a structural equation model to analyze the mediating effect of mental resilience between social support and fear of childbirth. Bias -corrected Bootstrap method was applied to test the mediating effect of mental resilience. Results: The total scores of social support of 186 primiparas in late-pregnancy was 47.62±7.20. The total score of mental resilience was 23.36 ±6.10; The total score of fear of childbirth was 35.66 ±6.62. Pearson correlation showed that social support was positively correlated with mental resilience (r=0.665, P<0.001), and negatively correlated with fear of childbirth (r=-0.478, P<0.001). Mental resilience was negatively correlated with fear of childbirth (r= -0.510, P<0.001). The test results of Bias-Corrected Bootstrap method showed that mental resilience had a partial mediating effect between social support and fear of childbirth (P<0.001), in which the mediating effect was -0.143, the direct effect was -0.308, and the total effect was -0.451, accounting for 31.71% of the total effect. Conclusion: Mental resilience has a mediating effect between social support and fear of childbirth of primiparas in the late-pregnancy. Clinical staff can alleviate the fear of childbirth by improving the level of mental resilience and social support.
