乳腺癌患者基本心理需求潜在剖面及影响因素分析

Latent profiles and influencing factors of basic psychological needs in breast cancer patients

摘要: 目的:分析乳腺癌患者基本心理需求的潜在类别特征及影响因素。方法:采用便利抽样法,选取 2024 年 7 月至 12 月山东省某三级甲等医院的乳腺癌患者作为研究对象。使用一般资料调查表、基本心理需求量表、社会支持量表、乳腺癌生活质量量表、自我效能量表进行调查,运用潜在剖面探究基本心理需求的类别及特征,采用多元 Logistic 回归分析相关因素。结果:220 例乳腺癌患者基本心理需求分为“低心理需求-低能力组”(占 18.2%)、“中心理需求-高压力组”(占 43.6%)、“高心理需求-关爱组”(占38.2%)3 个类别。多元 Logistic 回归分析显示,居住地、主要照顾者、月收入、生活质量水平、社会支持水平、自我效能感是乳腺癌患者基本心理需求的影响因素(P<0.05)。结论:乳腺癌患者基本心理需求水平有待提升且存在明显异质性,建议医护人员根据患者基本心理需求潜在剖面特点制定针对性策略,更好地满足患者的基本心理需求。

Abstract: Objective: To analyze the latent class characteristics and influencing factors of basic psychological needs in breast cancer patients. Methods: A convenience sampling method was adopted to recruit breast cancer patients from a tertiary grade A hospital in Shandong Province between July and December 2024. Data were collected using a general information questionnaire, the Basic Psychological Needs Scale (BPNS), the Social Support Rating Scale (SSRS), the Functional Assessment of Cancer Therapy-Breast (FACT-B), and the General Self-Efficacy Scale (GSES). Latent profile analysis (LPA) was performed to identify the latent classes and characteristics of basic psychological needs, and multivariate logistic regression analysis was conducted to examine the related influencing factors. Results: The basic psychological needs of 220 breast cancer patients were classified into three categories, including "low psychological needs-low competence group" (18.2%), "moderate psychological needs-high pressure group" (43.6%), and "high psychological needs-care group" (38.2%). Multivariate logistic regression analysis showed that place of residence, primary caregiver, monthly income, quality of life level, social support level, and self-efficacy were influencing factors of basic psychological needs in breast cancer patients (P<0.05). Conclusion: The level of basic psychological needs among breast cancer patients requires further improvement and exhibits significant heterogeneity. Therefore, it is recommended that healthcare providers develop targeted intervention strategies based on the latent profile characteristics of patients' basic psychological needs, in order to better address and fulfill their basic psychological needs.
