Application of multi-sensory support model based on perception theory in COPD patients with type II respiratory failure

摘要: 目的:探讨基于感知理论的多感官支持模式在慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭患者中的应用效果。方法:采用便利抽样法,选取天津市某三级甲等综合医院呼吸 ICU 于 2023 年 7 月至 2024 年 4 月收治的 87 例患者为研究对象,分为对照组(n=43)和 干预组(n=44)。对照组采用常规护理模式;干预组在常规护理的基础上采用多感官支持模式。比较两组患者干预前后睡眠质量、认知功能、谵妄发生率及血气分析指标水平。结果:干预后,干预组 Richards Campbell 睡眠量表评分、认知功能评分均高于对照组,谵妄发生率、谵妄持续时间、二氧化碳分压均低于对照组,差异有统计学意义(P<0.05)。结论:基于感知理论的多感官支持模式可有效改善 COPD 合并Ⅱ型呼吸衰竭患者的睡眠质量,减少谵妄的发生率及持续时间,改善患者认知状态。

Abstract: Objective: To explore the effect of multi-sensory support model based on perception theory in COPD patients with type II respiratory failure. Methods: A total of 87 patients admitted to the respiratory ICU of a grade A tertiary general hospital in Tianjin from July 2023 to April 2024 were selected by convenience sampling method and divided into control group (n=43) and experimental group (n=44). The control group received routine nursing model. The experimental group adopted multi-sensory support model on the basis of routine nursing. Sleep quality, cognitive function, incidence of delirium and blood gas analysis were compared before and after intervention between two groups. Results: After intervention, Richards Campbell sleep scale score and cognitive function score of experimental group were significantly higher than those of control group (P<0.05). The incidence of delirium, delirium duration and the PCO2 in experimental group were significantly lower than those in control group (P<0.05). Conclusion: The application of multi-sensory support model based on perception theory in COPD patients with type Ⅱ respiratory failure has good effects, which can effectively optimize the sleep structure of patients, reduce the incidence and duration of delirium, improve the cognitive status of patients.
