Influencing factors of kinesiophobia in pregnant women with pregnancy-related low back pain and/or pelvic pain

Abstract: Objective: To investigate the current status of kinesiophobia in pregnant women with pregnancy-related low back pain and/or pelvic pain (PR-LBPP), and analyze its influencing factors. Methods: The convenience sampling method was used and pregnant women with PR-LBPP who were examined in the obstetrics department from January 2022 to May 2023 were included. The general information questionnaire, Trampa scale for kinesiophobia (TSK), numerical rating scale (NRS), the pregnancy exercise self-efficacy scale(P-ESES) and family adaptation partnership growth affection and resolve index (APGAR) were used to investigate the pregnant women. Pearson correlation was used to analyze the correlation between TSK score and P-ESES and APGAR score. Multiple linear regression was used to analyze the influencing factors of exercisefear level in pregnant women. Results: The TSK score of 171 pregnant women was 37.15±6.91. Multiple linear regression analysis showed that age, primipara, gestational age, work during pregnancy, moderately severe pain, APGAR score and PESES score were the influencing factors of kinesiophobia of pregnant women (P<0.05), which could jointly explain 61.8% of the variation of kinesiophobia level. Conclusion: The level of kinesiophobia in pregnant women with PR-LBPP needs to be reduced, and the level is affected by many factors. Medical staff should pay attention to their exercise fear assessment and take targeted intervention measures as soon as possible to reduce exercise fear and promote physical activity and physical and mental health of pregnant women.

摘要: 目的:调查妊娠相关腰痛和/或骨盆痛(Pregnancy-related Low Back Pain and/or Pelvic Pain,PR-LBPP)孕妇运动恐惧现状,并分析其影响因素。方法:采用便利抽样法,选取 2022 年 1 月至 2023 年 5 月产科 PR-LBPP 孕妇 175 名。采用一般资料调查表、恐动症 Tampa 评分量表(Trampa ScaIe for Kinesiophobia,TSK)、疼痛数字评价量表(Numerical Rating Scale,NRS)、孕妇运动自我效能量表(the Pregnancy Exercise Self-efficacy Scale,P-ESES)及家庭关怀度指数量表(Family Adaptation Partnership Growth Affection and Resolve Index,APGAR)进行调查。采用 Pearson 相关分析 TSK 评分与 P-ESES、APGAR 评分的相关性;采用多元线性回归分析孕妇运动恐惧水平的影响因素。结果:共回收有效问卷 171 份。171 名孕妇 TSK 评分为(37.15±6.91)分。多元线性回归分析显示,年龄、分娩史、孕周、孕期工作、疼痛程度、APGAR 评分及 P-ESES 评分是孕妇运动恐惧水平的影响因素(P<0.05),可共同解释运动恐惧水平 61.8%的变异度。结论:PR-LBPP 孕妇运动恐惧水平有待降低,受多因素影响。医护人员应重视其运动恐惧评估,并尽早采取针对性干预措施,以减轻运动恐惧,促进孕妇身体活动与身心健康。
