The status and influencing factors analysis of thoracic surgery nurses′ knowledge, attitude, and practice in the care of swallowing disorders in patients undergoing esophageal cancer radical surgery

胸外科护士对食管癌根治术后患者吞咽障碍护理的知信行现状及影响因素分析

Abstract: Objective: To investigate thoracic surgery nurses′ knowledge, attitude, and practice regarding the care of swallowing dysfunction in patients after esophageal cancer radical surgery, and to analyze the influencing factors. Methods: A self-developed questionnaire on thoracic surgery nurses′ knowledge, attitude, and practice regarding the care of swallowing dysfunction in patients after esophageal cancer radical surgery was used to survey 319 thoracic surgery nurses from 24 comprehensive hospitals in Hubei Province. Results: The scores of thoracic surgery nurses on the three dimensions of knowledge, attitude, and practice regarding the care of swallowing dysfunction in patients after esophageal cancer radical surgery were 54.25±15.00, 51.04±7.05, and 44.14±9.14, respectively. Multiple linear stepwise regression analysis revealed that educational level, years of experience in thoracic surgery department, and specialized training in rehabilitation nursing were influencing factors for nurses′ knowledge(P<0.05). Dysphagia related training experience was the influencing factor of nurses′ attitude towards dysphagia care of patients after radical resection of esophageal cancer(P<0.05). Dysphagia related training experience and years of experience in thoracic surgery department were the main influencing factors of nurses′ behavior of dysphagia care in patients after radical resection of esophageal cancer(P<0.05). Conclusion: Nurses in thoracic surgery department generally have a positive attitude toward the care of swallowing dysfunction in patients after esophageal cancer radical surgery, but their knowledge level and practical behavior are insufficient. Clinical settings should develop targeted training programs based on the different characteristics of thoracic surgery nurses to enhance their intrinsic motivation and enthusiasm, thereby improving their nursing practices. Based on the different characteristics of thoracic surgery nurses, targeted training should be developed to enhance their intrinsic motivation and improve their nursing practice behavior.

摘要: 目的:调查胸外科护士对食管癌根治术后患者吞咽障碍护理的知识、态度、行为现状,并分析其影响因素。方法:采用自行编制的胸外科护士对食管癌根治术后患者吞咽障碍护理知信行调查问卷,以问卷星的形式对湖北省 24 家综合医院的 319 名胸外科护士进行调查。结果:胸外科护士对食管癌根治术后患者吞咽障碍护理知识、态度、行为 3 个维度的得分分别为(54.25±15.00)分、(51.04±7.05)分、(44.14±9.14)分。多元线性逐步回归分析结果显示,学历、胸外科工作年限及康复专科护士学习经历是护士护理知识的影响因素(P<0.05);吞咽障碍相关培训经历是护士护理态度的影响因素(P<0.05);吞咽障碍相关培训经历及胸外科工作年限是护士护理行为的影响因素(P<0.05)。结论:胸外科护士对食管癌根治术后患者吞咽障碍护理的态度较为积极,但知识水平与实践行为欠佳。临床应根据胸外科护士的不同特征,制定有针对性的培训,调动其内在驱动力及积极性,促进其护理实践行为。
