Effects of experiential childbirth-simulation education on primipara: a meta-analysis

Objective: To systematically evaluate the effects of experiential childbirth-simulation education on primipara.Methods: PubMed, Web of Science, Cochrane Library, CBM, CNKI and WanFang Data were used as the sources of literatures ,and all randomized controlled trials(RCTs) which studied the effects of experiential childbirth-simulation education on primipara were extracted from inception to July 10th, 2020. The meta-analysis was conducted using RevMan 5.3 software. Results: A total of 26 literatures involving 5 957 participants were included. The results of meta-analysis showed that experiential childbirth -simulation education could significantly increase the rate of natural delivery for primipara( OR=2.71, 95% CI: 2.40~3.05, P<0.01), decrease the incidence of postpartum hemorrhage( OR=0.11, 95% CI: 0.04~0.33, P<0.01) and neonatal asphyxia ( OR=0.23, 95% CI: 0.12~0.44, P<0.01), meanwhile improve the anxiety ( SMD=-1.68, 95% CI: -2.01~-1.35, P<0.01), depression( SMD=-1.37, 95% CI: -1.58~-1.15, P<0.01), and self-efficacy( MD= 5.60, 95% CI:2.92~8.28, P<0.01) of primipara. Conclusion: Experiential childbirth-simulation education is efficacious in increasing the rate of natural delivery for primipara, decreasing the incidence of postpartum hemorrhage and neonatal asphyxia, and improving the anxiety, depression and self-efficacy of primipara. Because of the limitation of quality of included studies, more high-quality studies are needed to confirm the above conclusion.

Qualitative study on the real experience of mother-infant skin contact in puerpera with gestational diabetes mellitus

Objective: To understand the real-life experience of mother-infant skin-to-skin contact in puerpera with gestational diabetes mellitus after the delivery of the newborn, and to identify the problems and needs of puerpera receiving skin-to-skin contact. Methods: A purposive sampling method was used,and 12 puerpera with gestational diabetes mellitus who delivered vaginally or underwent caesarean section in the obstetrics department of a class A tertiary hospital in Nanning were included from April to July 2022 for a semi-structured in-depth interview. Content analysis was used to analyze and distil the themes. Results: Three themes were identified including lack of knowledge and experience of the unknown, increased sense of adaptation and security,and benefits of knowledge and perceived of early exposure. Conclusion: Women with gestational diabetes mellitus have complex psychological experiences and needs when receiving mother-infant skin-to-skin contact. It is recommended that health care professionals develop professional, individualized interventions to promote maternal awareness and willingness to engage in postnatal mother-infant skin-to-skin contact and reduce the incidence of neonatal hypothermia and hypoglycemia.

The experience of maternal epidural analgesia delivery: a systematic review of qualitative studies

Objective: To systematically review qualitative studies on the experience of maternal epidural delivery analgesia, so as to provide reference basis for clinical improvement of labor analgesia management and obstetric medical service . Methods: PubMed, Web of Science, PsycINFO, Embase, CNKI, CBM, WanFang, VIP databases were retrieved from inception to June 2022,and literatures on qualitative studies of the maternal epidural analgesia delivery were extracted. The JBI critical appraisal tool for qualitative studies in Australia (2016 edition) was used to evaluate the quality of studies.The results were syn-the sized by using integration methods. Results: A total of 14 studies were included,and 46 research results were extracted. 3 integrated results were grouped from 7 categories according to their similarities, covering the decision making experience of maternal epidural analgesia delivery, the physical and mental experience of maternal epidural analgesia delivery, maternal knowledge and emotional needs. Conclusion: Nursing staff should fully focus on the plight of maternal epidural analgesia childbirth decisions, attaches great importance to the active birth experience, meet the demand of knowledge and the emotion, and strengthen birth experience.
