Application of active cycle of breathing techniques training based on the IKAP theory in patients with high-flow nasal oxygen therapy during ICU transition period

Objective: To explore the impact of active cycle breathing techniques training based on IKAP theory on dyspnea belief, peak expiratory flow rate, and exercise self-efficacy in patients with undergoing high-flow nasal oxygen therapy during ICU transition period. Methods: Patients with high-flow nasal oxygen therapy during ICU transition period were selected from a Class A tertiary hospital in Tianjin. The control group comprised 46 patients admitted from October 2021 to February 2022, who received active cycle breathing techniques training under conventional health education guidance. The observation group included 46 patients admitted from March to September 2022, who received active cycle breathing techniques training guided by the IKAP theory. The scores for dyspnea belief, peak expiratory flow rate, and exercise self-efficacy were compared between the two groups before and after the intervention. Results: The observation group had significantly lower scores for dyspnea belief, and higher scores for peak expiratory flow rate and exercise self-efficacy compared to the control group ( P<0.05). Conclusion: Active cycle breathing techniques training based on the IKAP theory can reduce patients′ exercise-related fear of dyspnea, enhance their ability to cough and expectorate, and improve their confidence in physical activity.

Construction,reliability and validity test of discharge preparation scale for patients with varicose veins of lower extremity after radiofrequency ablation surgery

Objective: To develop an instrument for measuring the discharge preparation for patients with varicose veins of lower extremity after radiofrequency ablation surgery and test its reliability and validity. Methods: Under the guidance of transition theory, the dimensions and entry pool of the scale were initially set through comprehensive literature search, screening, reading and group discussion. Items were modified through expert letter inquiry methods, pre-surveys, project analysis and exploration sub-analyses, and then formal scales were formed. Using the convenient sampling method, 176 patients who underwent varicose veins of lower extremity radiofrequency ablation surgery in the vascular ward of general surgery in a Class A tertiary hospital in Tianjin from July 2022 to April 2023 were included and the reliability and validity test were conducted to construct the discharge readiness scale for patients with varicose veins after surgery. Results: The discharge preparation scale for patients with varicose veins of lower extremity after radiofrequency ablation surgery included 22 items in 4 dimensions. They were disease knowledge, personal status, coping ability and social support. The cumulative variance contribution rate was 75.853%. The recovery rate of the two rounds of expert letters was 100%, the expert authority coefficients were 0.83 and 0.86, and Kendall coordination coefficients were 0.433 and 0.351 ( P<0.05). The total Cronbach′s α coefficient of the questionnaire was 0.954 and the Cronbach′s α coefficients of each dimension were 0.850, 0.958, 0.602 and 0.709. The content validity index (S-CVI) of the scale level was 0.962, and the content validity index (I-CVI) of the items level were 0.813 to 1.000. Conclusion: The discharge preparation scale for patients with varicose veins of lower extremity after radiofrequency ablation surgery constructed in this study is scientific and practical, and can be used to evaluate the discharge preparation of patients with varicose veins of lower extremity after radiofrequency ablation surgery.

The status and influencing factors of oral self-efficacy in elderly patients with chronic periodontitis

Objective: To investigate the current status of oral health self -efficacy in elderly patients with chronic periodontitis and to analyze its influencing factors. Methods: A convenience sampling method was used to investigate the oral health self-efficacy in 288 elderly chronic periodontitis patients by applying the General Situation Questionnaire, the Chinese version of Geriatric Self-Efficacy Scale for Oral Health (GSEOH) and the Dental Fear Scale (DFS). Results: The GSEOH score of elderly patients with chronic periodontitis was 57.84±4.28 and the DFS score was 37.69±9.78. The level of dental fear in elderly patients with chronic periodontitis was negatively correlated with the total score of GSEOH ( r=-0.457, P <0.001). The multiple regression analysis showed that age, education level, monthly income, residence, severity of periodontitis, and the level of dental fear were the influencing factors of self -efficacy in elderly patients with chronic periodontitis ( P<0.05). Conclusion: The level of oral health self-efficacy of elderly patients with chronic periodontitis needs to be improved, and nurses should pay attention to assessing the relevant influencing factors of the patients and adopting targeted nursing interventions to enhance their oral health self-efficacy.

Experience and demand of virtual reality analgesia for patients after resection for lung cancer: a qualitative research

Objective: To explore the experience and demands of patients after resection for lung cancer undergoing analgesia with virtual reality (VR) technology, and to provide reference for the development of relevant nursing protocols. Methods: A purposive sampling method was used to select 26 patients after resection for lung cancer who participated in VR-based analgesia. From June to September 2023, face-to-face semi-structured interviews were conducted, and Colaizzi′s phenomenological analysis was used to analyze the interview data. Results: The experience and demands of patients after resection for lung cancer using VR analgesia were summarized into 3 themes, including VR analgesia positive experience (recognition of VR analgesia effect and promotion of mood improvement), VR negative experience of analgesia (fear of experiencing new technology and discomfort during use), postoperative needs for VR analgesia (desire for enriched VR content and enhanced VR technology, the demand for VR operation guidance, the demand for a quiet environment and safety concerns, communication needs). Conclusion: VR analgesia presents positive analgesia effect and experience for patients after lung cancer resection. However, there are still some unmet service needs. Healthcare providers should explore coping strategies according to patients′ demands and perfect the nursing of VR analgesia for patients after resection for lung cancer.

The status and influencing factors of self-acceptance in patients with modified radical mastectomy

Objective: To investigate the relationship between self-acceptance and coping style of patients with modified radical mastectomy, and explore the influencing factors of self-acceptance. Methods: Self-acceptance questionnaire and medical coping style questionnaire were used to investigate the self-acceptance and coping style in 190 patients with modified radical mastectomy. Pearson correlation analysis was used to explore the correlation between coping score and self-acceptance score. Multiple linear regression was used to analyze the influencing factors of self-acceptance among patients with modified radical mastectomy. Results: The total score of self-acceptance in patients with modified radical mastectomy was 38.76±4.77. The facing coping style was positively correlated with self-acceptance, while the avoidance coping style was negatively correlated with self-acceptance( P<0.05). The results of regression analysis showed that the influencing factors of self-acceptance were average monthly household income, whether it was triple-negative, coping style of facing and avoiding( P<0.05). Conclusion: The self-acceptance level of patients with modified radical mastectomy needs to be improved. Helping patients with modified radical mastectomy adopt more facing coping styles and reducing avoidance coping styles can improve their self-acceptance level.

The real experience of role transformation from nurse into family in oncology: a qualitative study

Objective: To explore the real experience and to understand the needs, dilemmas, experiences and gains of oncology nurses as family members of cancer patients, providing reference for better balance between clinical work and family life. Methods: A total of 17 clinical nurses which had family members with cancer patients were selected through purposive sampling. The data were collected via a semi-structured interviews and analyzed by using Colaizzi′s 7-step phenomenological method from March 2019 to October 2022 in a cancer hospital in Tianjin, China. The organizing of interview data was performed by Nvivo 10.0. Results: Three themes were summarized from the real experience of oncology nurses as family members with cancer patients, including self-emotional experience(pain and sorrow, self-reproach and remorse, and calm and positive confrontation), increased burden of life (increased financial pressure, insufficient caregiving manpower, fertility worries, and changes in lifestyle and social activities), and sense of professional benefits(professional perception, professional resources, professional identity, and self -growth). Conclusion: The great pressure generated among oncology nurses which had dual role as nursing staff and family members with cancer patients. It′s indicated that support from work, family, society and many other aspects should be provided, to promote their well-developed both in mind and body.

The effect of family member synchronous empowerment education on self-perceived burden and quality of life in patients with esophageal cancer undergoing radiotherapy

Objective: To explore the influence of family members′ synchronous empowerment education on self-perceived burden and quality of life in patients with esophageal cancer undergoing radiotherapy, and to provide reference for the clinical implementation of family-based health education. Methods: A total of 82 patients with esophageal cancer who met the standard of radiotherapy were included from April 2021 to April 2022. Patients were divided into control group and observation group according to admission time. In the control group( n=41), patients were given empowerment education during radiotherapy. The observation group( n=41) was implemented synchronous empowerment education for family members and patients. The intervention time was from one day before radiotherapy to the end of radiotherapy. Before radiotherapy, at the end of radiotherapy, and one month after radiotherapy, patients with esophageal cancer were evaluated with the Self-Perceived Burden Scale (SPBS) and the EORTC Quality of Life Questionnaire-Core 30(EORTC QOL-C30). Results: At the end of radiotherapy and one month after radiotherapy, the total score of SPBS and the scores of physical, economic and emotional dimensions in the control group were higher than those in the observation group( P<0.05). The scores of QOL in the control group were lower than those in the observation group( P<0.05). Conclusion: Nurses should pay attention to the physical and mental health of family members in clinic, empower family members synchronously and improve their nursing ability, which is conducive to reducing the burden of patients′ self -feeling and improving their quality of life.

A qualitative study the real experience of prone ventilation therapy in conscious patients

Objective: To gain an in-depth understanding of the real experiences of prone ventilation therapy in conscious patients, and to provide a theoretical basis for healthcare professionals to develop targeted interventions. Methods: Using a purposive sampling method, semi-structured interviews were conducted from January 28 to February 18, 2023 among 15 conscious patients receiving prone ventilation therapy in the respiratory department of a Class A tertiary hospital in Yantai, and the data were coded, classified and refined into themes using the Colaizzi 7-step content analysis method. Results: The real experiences of the 15 conscious patients treated with prone ventilation could be categorized into 5 themes and 13 sub-themes, they were overload(somatic overload and psychological overload), attitude change(questioning the effect, increased sense of benefit and strong belief in treatment), insufficient support (family support, medical support and peer support), expectation better treatment (expectation of improved treatment process, expectation of systematic and standardized specialist guidance) and insufficient cognition (lack of knowledge, role cognitive bias and blind confidence in treatment outcome). Conclusion: Conscious patients treated with prone ventilation should have their measures reducing related stress load, and relevant measures should be taken to promote attitude change and confidence, so as to improve the effect of prone ventilation treatment. Enhancing patients′ cognitive level and providing systematic social support to improve their compliance with prone ventilation therapy are needed. We also improve the implementation process to improve the safety of prone ventilation treatment and give patients systematic and standardized specialist guidance to promote disease recovery.

Study on the correlation between social alienation and stigma in young patients with cervical cancer undergoing radiotherapy

Objective: To investigate the current situation of social alienation and stigma in young patients with cervical cancer undergoing radiotherapy, and explore the relationship between social alienation and stigma. Methods: Totally 175 cervical cancer patients were investigated by general information questionnaire, general alienation scale and social impact scale from February 2022 to December 2022. Results: The score of social alienation of young patients with cervical cancer treated with radiotherapy was 44.38±8.64, and the score of stigma was 76.36±10.17, which was positively correlated ( r= 0.672, P<0.01). Conclusion: Young patients with cervical cancer undergoing radiotherapy have a higher sense of social alienation. There is a positive correlation between the level of stigma and social alienation. Nurses should fully understand the thoughts of young patients with cervical cancer undergoing radiotherapy, do a good job of psychological counseling, help them accept their current situation, adapt to the changes in their roles, alleviate the level of stigma of patients, and help them better integrate into society.

Objective: The study deeply understands gastroenterology nurses′ cognition of adolescent patients′ transition readiness of inflammatory bowel disease, so as to providing a basis for managers to develop effective measures to improve nurses′ cognition of adolescent patients′ transition readiness and promote the further improvement and implementation of transition readiness nursing. Methods: Objective sampling method was used to conduct semi -structured in -depth interviews with 14 nurses nursing inflammatory bowel disease in the gastroenterology department in two Class A tertiary hospitals in Shanghai. The interview data were explored by Colaizzi 7-step analysis method. Results: Four themes were identified including recognition of the need for transition readiness, lack of transition readiness knowledge, poor implementation of transition readiness assessments and inadequate management of the transition readiness process. Conclusion: Gastroenterology nurses attach importance to the transition readiness of adolescent patients with inflammatory bowel disease, but their cognition and practice have limitations. Nursing managers should adopt scientific methods to improve nurses′ cognition and behavioral practice, accurately evaluate the transition readiness of adolescent patients, and implement relevant management norms to promote the smooth transition of adolescent patients to adults.

Current situation and influencing factors of continuous service needs in patients with cervical spondylosis after surgery

Objective: To analyze the status and influencing factors of continuous service needs in patients with cervical spondylosis after surgery. Methods: A total of 92 patients with cervical spondylosis were selected as the research objects. A self-made continuous service needs questionnaire was used to investigate the patients′ postoperative continuous care needs 1 day before discharge, and the status of patients with different characteristics was compared to identify the relevant influencing factors. Results: The score of continuous service needs was 40.35±5.76. There were statistically significant differences in the scores of postoperative continuous service needs of patients with cervical spondylosis in different age groups, educational level, postoperative hospital stay and neck dysfunction ( P<0.05). Multiple linear regression analysis showed that age, educational level, postoperative hospital stay and neck dysfunction were influencing factors of continuous service needs in patients with cervical spondylosis after surgery ( P<0.05). Conclusion: Patients with cervical spondylosis have high needs for continuous service after surgery, which is affected by the age of patients, educational level, postoperative hospital stay, and the dysfunction of neck.
