摘要:  

目的: 探讨袋鼠式护理对新生儿疼痛反应和体格发育的影响。方法: 选取自然分娩的足月新生儿100例,按照分段随机分组方法均分为对照组和观察组各50例。对照组新生儿按常规护理方式护理,观察组新生儿在常规护理的基础上自分娩后5 h内即接受袋鼠式护理1 h,并在随后住院的3天内,每天进行2次袋鼠式护理。出院后在家中继续实施袋鼠式护理至30天时。对两组新生儿进行足跟采血,测定采血前、采血时和采血后的心率和血氧饱和度。测量两组新生儿刚出生时和30天时的身高、体重和头围,并计算增长差值。结果: 足跟采血时和采血后观察组新生儿的心率低于对照组,而血氧饱和度高于对照组,两组差异具有统计学意义(P<0.01);观察组新生儿的身高、体重和头围的增长差值均要优于对照组,两组差异具有统计学意义(P<0.01); 结论:袋鼠式护理能缓解新生儿疼痛反应和促进新生儿体格发育,作为一种科学、有效和人性化的护理模式,袋鼠式护理值得在临床推广和应用。

Abstract:  

Objective: To explore the effects of kangaroo mother care on pain response and physical development of full-term newborns. Methods: 100 cases of full-term newborns delivered vaginally were selected as the research objects and randomly divided into a control group and an observation group 50 cases for each group. The control group was given routine care, while the observation group was given kangaroo mother care on the basis of routine care. In addition to 1h of kangaroo mother care was given the maternal from 5h after birth in the observation group, and within 3 days of hospitalization until 30 days after birth twice daily. The newborns of observation group were given kangaroo mother care 30 minutes before heel-stick until 10 minutes after collecting blood. The control group experienced the same heel-stick and received the standard hospital care only. Throughout the heel-stick procedure, heart rate and oxygen saturation were monitored. The height, body weight and head circumference of newborns 30 days after birth of two groups were recorded. Results: During heel-stick, heart rate was significantly reduced in the observation group compared with that of the control group (P< 0.01), while oxygen saturation was significantly higher in the observation group than that of the control group (P< 0.01). The increases of height, weight and head circumference of observation group 30 days after birth were significant higher than those of control group (P<0.01). Conclusion: Kangaroo mother care can alleviate the pain response and promote the physical development for full-term newborns, so it is worth to be widely used in clinical practice.
