母乳在管饲喂养早产儿口腔护理中的应用效果评价

目的:观察用母乳进行口腔护理对管饲喂养早产儿口腔清洁度喂养不耐受及早产儿生长发育的影响。方法:将2015年2月至2016年6月收治的95例管饲喂养早产儿(其中5例患儿家属要求自动出院),根据置管时间的先后顺序分为单双号,其中单号为观察组,双号为对照组;观察组45例,选用母乳进行口腔护理;对照组45例,选用0.9%氯化钠溶液进行口腔护理。比较两组管饲喂养早产儿口腔清洁度、口腔感染发生率、喂养不耐受及坏死性小肠结肠炎(NEC)发生率、胃管留置时间、矫正胎龄37周体重增长百分比。结果:观察组管饲喂养早产儿矫正胎龄37周体重增长百分比、口腔清洁度明显高于对照组,且观察组管饲喂养早产儿口腔感染发生率、喂养不耐受发生率、NEC发生率、胃管留置时间均低于对照组,两组数据比较差异具有统计学意义(P<0.05)。结论:母乳口腔护理应用于管饲喂养早产儿可有效清洁口腔,减少口腔感染。微量母乳遗留于口腔可通过早产儿吞咽动作进入胃肠道,促进其胃肠道功能发育成熟,增强其抵抗力,缩短从管饲喂养过渡到经口足量喂养时间,减少喂养不耐受及NEC发生。

Abstract:  

Objective: To observe the effects of breast milk on oral care for premature infants feeding by tube. Methods: 90 premature infants breastfeeding by tube from February 2015 to June 2016 were divided into treatment group and control group according to indwelling number. The 45 cases in treatment group were cared with oral immunotherapy by breast milk, the other 45 cases in control group were by 0.9% sodium chloride solution. The oral cleanliness, incidence rate of oral infection, incidence rate of feeding intolerance and necrotizing enterocolitis(NEC), retention time of stomach tubes, increased percentage of fetus at 37w gestational age were compared between these two groups. Results: All clinical indexes in treatment group were obviously better than that of in control group(P<0.05). Conclusion: Oral care of breast milk can be applied to premature infants feeding by tube, so as to reduce oral infection rate. Slight breast milk in oral can get through into gastrointestinal tract with swallowing, it can make gastroenteric maturation, strengthen immunity, reduce the transitional time from tube feeding to mouth feeding sufficiently, reduce incidence rate of feeding intolerance and NEC for premature infants.
