基于目标管理的张口训练对鼻咽癌放疗患者营养相关症状群及生活质量的影响

摘要: 目的:探讨目标管理在鼻咽癌放疗患者张口训练中的应用效果。方法:选择2018年10月至2020年12月拟行放射治疗的鼻咽癌患者95例为研究对象,采用随机数字表法分为观察组48例、对照组47例。对照组给予常规护理干预及张口训练管理,观察组应用基于目标管理的张口训练。干预6个月,比较两组患者张口困难发生率、营养相关症状群、生活质量等指标。结果:观察组张口困难发生率、口腔/咽喉疼痛、吞咽/咀嚼困难、口腔和喉咙黏液、味觉异常、便秘、营养相关症状群总分低于对照组,躯体功能、心理功能、社会功能、共性症状与副作用、特异模块、生活质量总分高于对照组,差异有统计学意义(P<0.05)。结论:基于目标管理的张口训练有助于减少鼻咽癌放疗患者张口困难发生率,缓解营养相关症状负担,改善患者生活质量。

Abstract: Objective:To explore the effect of objective management in mouth opening training of patients with nasopharyngeal carcinoma undergoing radiotherapy. Methods: A total of 95 patients with nasopharyngeal carcinoma who planned to receive radiotherapy from October 2018 to December 2020 were included and were divided into the observation group(n=48) and the control group(n=47) by random number table method. The control group was given routine nursing intervention and mouth opening training management, and the observation group received mouth opening training combined objective management. After 6 months of intervention, the incidence of dysphagia, nutrition-related symptoms, quality of life and other indicators were compared between the two groups. Results: The incidence of mouth opening difficulty in the observation group was lower than that in the control group(P=0.023). The total scores of oral/throat pain, dysphagia/mastication, oral and throat mucus, abnormal taste, constipation and nutrition-related symptoms were lower than those in the control group. The total scores of physical function, psychological function, social function, common symptoms and side effects, specific modules and quality of life were higher than those in the control group. Conclusion: Mouth opening training based on objective management can reduce the incidence of mouth opening difficulty, relieve the burden of nutrition-related symptoms and improve the quality of life of patients with nasopharyngeal carcinoma undergoing radiotherapy.
