慢性肾脏病患者出院后延续性护理需求的质性研究

Qualitative study on the continuous nursing needs of patients with chronic kidney disease after discharge

摘要: 目的:了解慢性肾脏病(Chronic Kidney Disease,CKD)患者出院后的延续性护理需求,为其出院后延续性护理方案的制定提供依据。方法:采用现象学研究方法,于2021年1月至4月选取肾内科住院患者11例进行半结构式访谈,采用Colaizzi7步分析法整理分析资料。结果:共提炼5个主题,分别是肾脏病较隐匿,患者容易忽视;病因复杂,病情进展迅速;延续性护理知晓率低;延续性护理涉及学科多,患者知识缺乏;延续性护理服务形式较单一。结论:医护要重视CKD患者的管理,为患者搭建多学科、多模式的延续性护理服务平台,同时应进一步拓展延续性护理服务内涵。

Abstract: Objective: To explore the continuous nursing needs of chronic kidney disease (CKD) patients after discharge, providing evidence for the development of continuous nursing programs after discharge. Methods: A phenomenological research approach was employed. Eleven CKD patients who were hospitalized in the Department of Nephrology from January to April 2021 were selected for semi-structured interviews. Colaizzi′s seven-step analysis method was used to organize and analyze the data. Results: Five themes regarding the continuous nursing needs of CKD patients after discharge were identified, including the insidious nature of CKD, leading patients to neglect it; the complex etiology and rapid progression of the disease; low awareness of continuous nursing; the involvement of multiple disciplines in continuous nursing, with patients lacking relevant knowledge; and the relatively singular forms of continuous nursing services. Conclusion: Healthcare providers should pay more attention to the management of CKD patients, establish a multi-disciplinary and multi-modal continuous nursing platform for patients, and further expand the connotation of continuous nursing services.
