骨质疏松性椎体骨折患者术前深静脉血栓形成的危险因素分析

摘要: 目的:分析骨质疏松性椎体骨折患者术前发生深静脉血栓形成(Deep Vein Thrombosis,DVT)的危险因素。方法:回顾性分析 346 例骨质疏松性椎体骨折住院患者的临床资料,按照患者术前 DVT 的诊断结果将其分为 DVT 组(n=34)和非 DVT 组(n=312),采用单因素及多因素 Logistic 回归分析术前 DVT 的危险因素。结果:346 例骨质疏松性椎体骨折患者 DVT 发生率为 9.8%。Logistic 回归分析结果显示,身体质量指数(BMI)、D -二聚体(D-dimer)、C 反应蛋白(CRP)是骨质疏松性椎体骨折患者深静脉血栓形成的危险因素,D -二聚体具有较高的预测灵敏度和特异度。结论:BMI、D-dimer 和 CRP 水平与术前 DVT 的发生密切相关,关注这些指标变化有助于早期识别 DVT 高风险患者,以便及时采取预防和干预措施,优化患者的术前管理,减少并发症的产生。

Abstract: Objective: To analyze the risk factors for preoperative deep vein thrombosis (DVT) in patients with osteoporotic vertebral fractures. Methods: A retrospective analysis was conducted on clinical data from 346 hospitalized patients with osteoporotic vertebral fractures. Based on preoperative DVT diagnostic results, patients were divided into a DVT group (n= 34) and a non-DVT group (n=312). Univariate and multivariate logistic regression analyses were used to identify preoperativerisk factors for DVT. Results: The incidence of DVT among the 346 patients with osteoporotic vertebral fractures was 9.8%. Logistic regression analysis revealed that body mass index (BMI), D-dimer, and C-reactive protein (CRP) were significant risk factors for DVT. D-dimer demonstrated high predictive sensitivity and specificity. Conclusion: Elevated levels of BMI, D-dimer, and CRP are closely associated with preoperative DVT. Monitoring these indicators facilitates early identification of high-risk patients, enabling timely preventive and interventional measures to optimize preoperative management and reduce complications.
