基于配偶视角的肝癌长期生存者家庭经济负担体验与需求的质性研究

Family economic burden experiences and needs of long-term survivors of liver cancer: a qualitative study from their spouse′s perspective

摘要: 目的:基于配偶视角了解肝癌长期生存者的家庭经济负担与相关需求,以帮助患者及其配偶应对经济负担。 方法:运用描述性质性研究方法,对16名肝癌长期生存者的配偶进行半结构式深度访谈。采用主题分析法进行归纳分析并提炼主题。结果:共提炼出4个主题,11个亚主题。 分别为经济负担繁重(直接医疗费用支出“不堪重负”、间接经济开销“入不敷出”);经济负担的影响(患者的治疗依从性降低、配偶的心理负担加重、家庭生活质量降低);经济负担的应对策略(降低消费标准并减少支出、兼职工作增加收入、帮扶支持与申请救助);期望与需求(对医疗机构的需求、对医保政策的需求、对社会支持系统的需求)。结论:肝癌长期生存者及其配偶存在巨大家庭经济负担及负性心理体验,医护人员应关注此类人群经济负担的真实体验与需求,采取多途径的综合干预策略及措施,帮助肝癌长期生存者及其配偶有效应对经济负担。

Abstract: Objective: To understand the family economic burden and related needs of long-term liver cancer survivors from the perspective of their spouses, with the aim of assisting patients and their spouses in coping with financial stress. Methods: A descriptive qualitative research method was employed and semi-structured in-depth interviews with 16 spouses of long-term liver cancer survivors were conducted. Thematic analysis was used to inductively analyze the data and extract themes. Results: Four main themes and eleven sub-themes were identified including heavy economic burden (overwhelming direct medical expenses; insufficient income to cover indirect economic costs), impact of economic burden (reduced treatment adherence in patients; increased psychological stress for spouses; decline in family quality of life), coping strategies for economic burden (lowering consumption standards and reducing expenses; taking on part-time jobs and increasing income; seeking support and applying for financial assistance), expectations and needs (needs related to medical institutions; needs related to health insurance policies; needs related to social support systems). Conclusion: Long-term liver cancer survivors and their spouses face significant family economic burdens and negative psychological experiences. Healthcare professionals should pay attention to the real experiences and needs of this population regarding financial stress and implement multi-faceted, comprehensive intervention strategies and measures to help long-term liver cancer survivors and their spouses effectively cope with economic burdens.
