基于症状体验时间模型的胃癌患者康复轨迹的纵向质性研究

A longitudinal qualitative study on the rehabilitation trajectory of gastric cancer patients based on symptom experience time model

摘要: 目的:基于症状体验时间模型分析胃癌患者不同阶段的康复体验,为构建针对性的康复管理干预方案提供依据。方法:采用便利抽样法,选择 2024 年 1 月至 2025 年 9 月天津市某三级甲等肿瘤医院住院进行胃癌根治术的 18 例患者为研究对象。根据时机理论对患者进行 5 次半结构式访谈,采用解释现象学对资料进行分析、归纳并提炼主题。结果:共提炼出疾病诊断期认知冲击与应对茫然、住院治疗期创伤适应与症状耐受、出院准备期自我康复管理能力初建、调整康复期自我康复管理实践障碍突破、长期适应期生活角色与价值重塑共 5 个主题、15 个亚主题。结论:胃癌患者在不同康复阶段存在不同需求,术后症状群的多维度、动态演变及交互影响特性增加了康复管理的复杂性。临床护理人员应深入理解并把握胃癌患者康复本质和变化规律,制定更具针对性和个性化的康复管理措施,为胃癌术后患者的康复提供更全面的支持。

Abstract: Objective: Based on symptom experience time model, the study analyze gastric cancer patients' rehabilitation experiences at different stages, providing evidence for developing targeted rehabilitation management interventions. Methods: A total of 18 patients undergoing radical gastrectomy for gastric cancer were selected through convenience sampling method as study subjects from a tertiary grade A oncology hospital in Tianjin between January 2024 to September 2025. Five semi-structured interviews were conducted with patients based on the timing theory, and interpretive phenomenology was employed to analyze, synthesize, and extract themes from the data. Results: Five themes and 15 sub-themes were identified, including cognitive shock and coping confusion during the disease diagnosis phrase, trauma adaptation and symptom tolerance during hospitalization treatment, initial establishment of self-rehabilitation management capacity during discharge preparation, overcoming practical barriers in self-rehabilitation management during recovery adjustment, and reconstruction of life roles and values during long-term adaptation phase. Conclusion: Gastric cancer patients exhibit distinct rehabilitation needs at different stages. The multidimensional, dynamic evolution, and interactive influence of postoperative symptom clusters complicate rehabilitation management. Clinical caregivers should thoroughly understand the fundamental nature and changing patterns of gastric cancer recovery, develop more targeted and personalized rehabilitation strategies, and provide comprehensive support for postoperative patients' recovery.
