Study on the status of financial toxicity and its impact on quality of life in patients with breast cancer

乳腺癌患者经济毒性现状及其对生存质量影响的研究

Abstract: Objective: To explore the current status of financial toxicity in patients with breast cancer and its impact on quality of life. Methods: The general information questionnaire, the Comprehensive Score for Financial Toxicity based on the Patient-Report Outcome Measures (COST-PROM), and the Functional Assessment of Cancer Therapy-Breast (FACT-B) were used to investigate 210 patients with breast cancer hospitalized in the department of breast oncology in a Class A tertiary hospital in Tianjin. Results: Patients with breast cancer scored 18.00 (12.00, 23.00) points in financial toxicity and (77.99±22.65) points in quality of life. 65.7% of patients had financial toxicity. Spearman correlation analysis showed that the total score of financial toxicity was positively correlated with the total score of quality of life (r=0.567, P<0.01). Multiple linear regression analysis showed that chemotherapy and financial toxicity were the influencing factors of quality of life and explained 43.3% of the overall variation. Conclusion: The financial toxicity in patients with breast cancer is common. Chemotherapy and financial toxicity can significantly predict patients′ quality of life. Nurses can implement targeted measures to reduce chemotherapy-related adverse reactions and reduce the level of financial toxicity, thereby improving patients′ quality of life.

摘要: 目的:探讨乳腺癌患者经济毒性现状及其对生存质量的影响。方法:采用一般资料调查表、患者报告结局的经济毒性综合评分量表(COST-PROM)、乳腺癌患者生存质量测定量表(FACT-B)对天津市某三级甲等医院乳腺肿瘤内科住院的210 例乳腺癌患者进行调查。结果:乳腺癌患者经济毒性得分为18.00(12.00,23.00)分,65.7%的患者存在经济毒性;乳腺癌患者生存质量得分为 (77.99±22.65)分。Spearman 相关分析显示,经济毒性总分与生存质量总分呈正相关(r=0.567,P<0.01)。多元线性回归分析显示,是否化学治疗及经济毒性是生存质量的影响因素,可解释总体变异的43.3%。结论:乳腺癌患者普遍存在经济毒性,化学治疗、经济毒性可以显著预测其生存质量。护理工作者可通过实施针对性措施,减轻化疗相关不良反应,降低经济毒性水平,从而改善其生存质量。
