Analysis of the current status and influencing factors of self-management in young and middle-aged patients with preventive enterostomy

中青年预防性肠造口患者自我管理现状及影响因素分析

Abstract: Objective: To explore the current status of self-management among young and middle-aged patients with preventive colostomy and analyze influencing factors. Methods: A convenience sampling method was employed to select 175 young and middle-aged patients with preventive enterostomy who were receiving outpatient or inpatient care at three grade Ⅲ and first-class hospitals in Xuzhou from October 2023 to May 2024. Data were collected using a general information questionnaire and a self-management questionnaire for enterostomy patients. Multiple linear regression analysis was used to explore the influencing factors of self-management in these patients. Results: A total of 160 valid questionnaires were collected, with an effective response rate of 91.43%. The total score of self-management ability of young and middle-aged patients with preventive enterostomy was 88.71±8.73, indicating a moderate level of self-management. Multiple linear regression analysis revealed that education level, stoma location, time since stoma surgery, postoperative chemotherapy or radiotherapy, and stoma acceptance were significant factors influencing self-management levels(P<0.05). Conclusion: The self-management ability of young and middle-aged patients with preventive enterostomy needs to be improved. Healthcare professionals should pay particular attention to patients with low education, ileostomy, early postoperative stages, those who have undergone postoperative chemotherapy or radiotherapy, and those with poor stoma acceptance. Targeted interventions should be implemented to improve patients′ self-management knowledge and behaviors.

摘要: 目的:探讨中青年预防性肠造口患者自我管理的现状并分析其影响因素。方法:采用便利抽样法,于2023年10月至2024年5月,选取徐州市3家三级甲等医院门诊就医或病房住院的175例中青年预防性肠造口患者作为调查对象。采用一般资料调查表和肠造口患者自我管理问卷进行调查。采用多元线性回归分析探索中青年预防性肠造口患者自我管理水平的影响因素。结果:共回收160份有效问卷,有效问卷回收率为91.43%。中青年预防性肠造口患者自我管理能力总分为(88.71±8.73)分,处于中等水平。多元线性回归分析结果显示,文化程度、造口部位、造口术后时间、造口术后放化疗、造口接受度是影响中青年预防性肠造口患者自我管理水平的因素(P<0.05)。结论:中青年预防性肠造口患者自我管理能力有待提升,护理人员应重点关注文化程度低、回肠造口、造口术后早期、造口术后放化疗及造口接受度差的人群,采取针对性的改进措施改善患者的自我管理认知与行为。
