Construction of health management need assessment scale for elderly patients with chronic disease and its reliability and validity

Objective: To construct the health management need assessment scale for elderly patients with chronic diseases and test its reliability and validity. Methods: Based on the Omaha system, the first draft of the health management need scale for elderly patients with chronic diseases was formed through literature analysis, expert correspondence and preinvestigation. A questionnaire survey was conducted in elderly patients with chronic diseases in a Class A tertiary hospital in Tianjin from March to September 2022 to test the reliability and validity of the scale. Results: A total of 330 questionnaires were distributed and 320 valid questionnaires were recovered. The final scale consisted of 27 items in 4 dimensions including physical health need, psychosocial health need, environmental health need and health behavior related needs. Exploratory factor analysis showed that the four common factors explained 58.646% of the total variance. Cronbach′s α were 0.803 ~0.886 for subscales, and Cronbach’s α for the total scale was 0.917. Split -half reliability coefficients were 0.801~0.914 for subscales, and 0.815 for the total scale. Test-retest reliability coefficients were 0.498~ 0.647 for subscales, and 0.911 for the total scale. Conclusion: The health management need assessment scale for elderly patients with chronic diseases is scientific, reasonable, reliable and valid, which can be used to evaluate the health management needs of elderly patients with chronic diseases.

Application of LEARNS health education model in patients with type 2 diabetes

Objective: To explore the effect of LEARNS health education on metabolic indicators, enthusiasm and related problems in patients with type 2 diabetes. Methods: A total of 118 patients with type 2 diabetes were randomly divided into the observation group (60 cases) and the control group(58 cases). The observation group adopted the LEARNS health education model intervention, while the control group adopted routine health education. The metabolic indicators(glycosylated hemoglobin, fasting blood glucose, triglyceride and total cholesterol), patients’ enthusiasm and diabetes related problems were compared between the two groups before and after intervention. Results: After the intervention, the metabolic indicators of the patients in the observation group were superior to those in the control group, the scores of enthusiasm were higher than those in the control group, and the scores of diabetes related problems were lower than those in the control group, with a statistically significant difference( P<0.05). Conclusion:LEARNS health education model can improve the metabolic indicators of patients with type 2 diabetes, improve the enthusiasm of patients, reduce diabetes related problems, and alleviate the pressure and bad mood of patients with type 2 diabetes.

Study on the effect of the medical integration mode on readiness for discharge in patients underwent pancreaticoduodenectomy

Objective: To explore the effect of the medical integration mode on readiness for discharge and the quality of discharge guidance in patients and caregivers after pancreaticoduodenectomy. Methods: Convenience sampling was used and 40 inpatients who underwent pancreaticoduodenectomy in the department of pancreatic oncology were included in the intervention group from October 2020 to June 2021. Another 40 patients who underwent pancreaticoduodenectomy in the department of pancreatic oncology were included in the control group from January to September 2020. The control group received routine care and discharge guidance, and the intervention group received the medical integration mode intervention. The readiness for discharge and the quality of discharge guidance were compared between two groups. Results: The total scores of the readiness for discharge and the scores of all dimensions of the intervention group patients and caregivers were significantly higher than those of the control group ( P<0.05). The total scores of discharge guidance quality and the scores of all dimensions of the intervention group were significantly higher than the control group ( P<0.05). Conclusion: The medical integration mode can improve the readiness for discharge and the quality of discharge guidance in patients and caregivers underwent pancreaticoduodenectomy, which is effective for developing continuous nursing services after discharge.

Visual analysis of acupoint application in recent 10 years in China

Objective: To explore the research hotspot and trend in the field of acupoint application in the past 10 years. Methods: The relevant literature on acupoint application in CNKI database was searched. The author, institution and keyword atlas were drawn and analyzed. Results: A total of 5 426 papers were included, with a steady increase in the total number of papers, and presented two main stages. A total of 527 authors were included, the most published author was scholar Liu Lanying, and the most published institution was the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine. There were 8 keyword cluster structures. Research hotspots and frontiers mainly focus on the combined application of acupoint application and emotional nursing and other nursing methods, and the clinical application of acupoint application in different diseases. Conclusion: It is found that the future development trend of acupoint application research mainly lies in the intervention research on improving sleep quality and pain degree, strengthening the combined application of acupoint application and emotional care, and exploring the prevention of adverse reactions of acupoint application. Therefore, it is the future development direction of acupoint application to carry out multi-center, large sample, deep related direction research and internal mechanism exploration.

Analysis of the current situation and influencing factors of nurses' experience and support as the second victim of nursing adverse events

Objective: To understand the current situation of nurses’ experience and support as the second victim of adverse nursing events, and analyze the influencing factors. Methods: A total of 421 nurses who had experienced adverse nursing events in a hospital in Tianjin were included by convenience sampling method from January to March 2022. General data questionnaire, second victim experience and support scale and mental toughness scale were used for the survey, and the influencing factors were analyzed by multiple linear regression. Results: The total score of experience and support scale for nurses as the second victim of adverse nursing events was 91.74±12.66, and the total score of mental toughness scale for nurses who had experienced adverse nursing events was 90.12±17.58. There was no correlation between the second victim experience and the total score of support, the second victim total score of pain and mental resilience, and the scores of all dimensions( P>0.05). Organizational support was positively correlated with total score of mental toughness and self-strengthening ( P<0.05), while non-job-related support was positively correlated with total score of mental toughness, tenacity, optimism and self-strengthening( P<0.05). Multiple linear regression analysis showed that educational level, professional rank and understanding of the concept of "second victim" were the influencing factors of the pain of nurses as the second victim of adverse nursing events ( P<0.05). The number of night shift per month and the total score of mental toughness were influencing factors of organizational support of nurses(P<0.05). Conclusion: The experience and support level of nurses as the second victim of adverse nursing events need to be improved. Adverse nursing events have a negative impact on the physical and mental well-being of nurses as the second victim, and the available support is short. It is suggested that nursing managers should adopt targeted methods to guide and help nurses to alleviate the negative effects of adverse events on nurses’ physical and mental health according to the characteristics of nurses, and improve the experience and support level of nurses as the second victim of adverse nursing events by improving the mental toughness level.

Multi-center investigation research on the status and influencing factors of cognition on early rehabilitation of critically ill patients among ICU nurses

Objective: To explore the status of cognition on early rehabilitation of critically ill patients and analyze its influencing factors among ICU nurses based on a multi-center survey. Methods: Nurses from 18 tertiary general hospital in Tianjin were included by convenience sampling method from November 2022 to December 2022. The general information questionnaire and ICU nurses’ early critical rehabilitation cognition questionnaire were used to investigate the cognition level of theoretical knowledge of early critical rehabilitation and the practical ability of rehabilitation technology in ICU nurses, and to explore the influencing factors of cognition on early rehabilitation of critically ill patients among ICU nurses. Results: The average score of ICU nurses’ overall cognition on early rehabilitation of critically ill patients was 3.86 ±0.63, and the average score of rehabilitation theoretical knowledge and rehabilitation technical practice ability were 3.98± 0.62 and 3.76±0.72, respectively. Multiple linear regression showed that job position, whether to receive training related to early rehabilitation, whether to have an early rehabilitation program in the department, whether to have a full -time rehabilitation nurse in the department were independent influencing factors of cognition on early rehabilitation of critically ill patients among ICU nurses. Conclusion: The overall cognition on early rehabilitation of critically ill patients is at a medium level in ICU nurses. It is necessary to further strengthen the training related to critical rehabilitation, improve the clinical practice ability of rehabilitation technology, optimize the staffing structure of rehabilitation specialist nurses, so as to build standardized practice protocols to promote the continuing development of critical rehabilitation.

The status of stress and post-competency and its influencing factors in head nurses

Objective: To analyze the current situation of stress and post-competency in head nurses,and to explore its influencing factors. Methods: A total of 495 head nurses were included, and the nurses’ job stressors scale and head nurses’ post-competency scale were used in the investigation. Results: The total score of stressors was 77.68±13.86,and the main sources of head nurses’ stress were workload and time allocation. The total score of the head nurses’ post-competency was 129.01±22.06, and the scores of personal attitude and traits were the highest. The tenure of the head nurses, hospital grade and department were the influencing factors of the stress in the head nurses. The tenure of the head nurses and education level were the influencing factors of the post-competency in the head nurses. Conclusion: Managers should pay more attention to the stress and post -competency in the head nurses. It is necessary to reduce the workload of head nurses, strengthen time management and scientific training program, so as to reduce the stress level and improve the post-competency in the head nurses.

Application of active cycle of breathing techniques training based on the IKAP theory in patients with high-flow nasal oxygen therapy during ICU transition period

Objective: To explore the impact of active cycle breathing techniques training based on IKAP theory on dyspnea belief, peak expiratory flow rate, and exercise self-efficacy in patients with undergoing high-flow nasal oxygen therapy during ICU transition period. Methods: Patients with high-flow nasal oxygen therapy during ICU transition period were selected from a Class A tertiary hospital in Tianjin. The control group comprised 46 patients admitted from October 2021 to February 2022, who received active cycle breathing techniques training under conventional health education guidance. The observation group included 46 patients admitted from March to September 2022, who received active cycle breathing techniques training guided by the IKAP theory. The scores for dyspnea belief, peak expiratory flow rate, and exercise self-efficacy were compared between the two groups before and after the intervention. Results: The observation group had significantly lower scores for dyspnea belief, and higher scores for peak expiratory flow rate and exercise self-efficacy compared to the control group ( P<0.05). Conclusion: Active cycle breathing techniques training based on the IKAP theory can reduce patients′ exercise-related fear of dyspnea, enhance their ability to cough and expectorate, and improve their confidence in physical activity.

Analysis on medication compliance of targeted therapy and influencing factors in patients with non-small cell lung carcinoma

Objective: To investigate the medication compliance of targeted therapy in patients with non-small cell lung carcinoma and analyze its influencing factors, so as to provide reference for improving the compliance of targeted therapy.Methods: A total of 208 patients with non-small cell lung carcinoma who received targeted therapy were included from February 2021 to July 2021 by convenient sampling method. The general information questionnaire, medication compliance scale, drug literacy questionnaire of discharged patients and social support evaluation scale were used for the survey.Results: The score of medication compliance of targeted therapy in patients with non-small cell lung carcinoma was 4.52±1.58, high compliance accounted for 2.9%, medium compliance accounted for 18.3%, and low compliance accounted for 78.8%. Multiple regression analysis showed that the influencing factors of the medication compliance of targeted therapy in patients with non-small cell lung carcinoma were drug literacy, social support, age,residence, and mode of residence.Conclusion: The compliance of targeted therapy in patients with non -small cell lung carcinoma is low. Medical staff should encourage family members to give more attention and support to patients, actively build a communication platform for patients with non-small cell lung carcinoma, enhance support among patients, further develop continuous nursing and provide medication guidance and consultation for discharged patients, so as to improve drug literacy and medication compliance of patients.

Current status and influencing factors of knowledge,attitude andpractice of palliative care amongnurses in Tianjin

Objective: To investigate the current status of knowledge, attitude and practice of palliative care among nurses in Tianjin and analyze the relevant influencing factors so as to provide reference for formulating the development strategy of palliative care in Tianjin. Methods: A cross-sectional survey design was used to investigate 9 953 nursing staffs from 73 hospitals in Tianjin by electronic questionnaires. The survey tools included general information questionaire, palliative care knowledge questionnaire, palliative care attitude questionnaire, and palliative care behavior questionnaire. Univariate and multiple linear regression analysis were used to analyze the influencing factors of knowledge, attitude and practice of palliative care. Results: The score of knowledge of palliative care was 9.56±3.27, and the scoring rate was 47.81%. The score of attitude of palliative care was 85.97±10.54 and the scoring rate was 68.78%. The score of behavior of palliative care was 46.89±10.93, and the scoring rate was 66.99%. The factors influencing the score of knowledge of palliative care among nursing staffs in Tianjin included hospital category, department, educational background, experience of nursing terminal patients and experience of nursing terminal relatives. Hospital level, hospital category, age, gender, working years,nursing hierarchy, experience of nursing terminal patients and experience of nursing terminal relatives were influencing factors of the score of attitude of palliative care. Department, age, experience of nursing terminal relatives were influencing factors of the score of behavior of palliative care among nursing staffs in Tianjin. Conclusion: The knowledge level of palliative care among nursing staffs in Tianjin is at a low level, the attitude of palliative care is not active, and the practice in palliative care behavior is inadequate. Training and other targeted measures should be taken to provide specialized talent reserves for promoting the development of palliative care in Tianjin.

The effect of the flash glucose monitoring system combined with exercise management on self-management behavior in patients with type 2 diabetes

Objective: To explore the effect of the flash glucose monitoring system combined with exercise management on self-management behavior in patients with type 2 diabetes. Methods: Patients with type 2 diabetes mellitus who were hospitalized in the endocrinology department of a class A tertiary hospital in Luoyang City from April 2019 to May 2020 were included. A total of 48 patients were divided into the intervention group and the control group with 24 in each group using the random number table method. The control group received flash glucose monitoring system combined with conventional diabetes health education and exercise instruction, while the intervention group received flash glucose monitoring system combined with exercise management based on conventional diabetes health education. The changes in self-management behavior, fasting glucose, glycated hemoglobin and lipid metabolism of patients were compared between two groups after 24 weeks. Results: After 24 weeks, the scores of three dimensions of the ability of self-management behavior including exercise, glucose monitoring and foot management in the intervention group were significantly higher than the control group ( P< 0.05). The results of total cholesterol, triglyceride, HDL cholesterol, and LDL cholesterol were in the intervention group were better than the control group, and the differences were statistically significant( P<0.05). Conclusion: Flash glucose monitoring system combined with exercise management can improve self-management behavior and modify lipid metabolism in patients with type 2 diabetes mellitus.

A visual analysis of research hotspots for pressure injury risk prediction based on machine learning algorithms

Objective: To analyze the research hotspots and development trends of pressure injury risk prediction based on machine learning algorithms by using bibliometric methods. Methods: CNKI, Wanfang, VIP and PubMed databases were retrieved from the inception of databases to October 31, 2022. The bibliographic co-occurrence analysis system software was used to extract the literature data. CiteSpace and gCLUTO software were used to visually cluster the literature data. Results:A total of 1 504 articles were retrieved, including 1 239 Chinese articles and 265 English articles. According to the inclusion and exclusion criteria, 89 articles were included, including 38 Chinese articles(42.70%) and 51 English articles (57.30%). The number of papers published in Chinese and English in this field showed an overall increasing trend. The included articles were published in 63 journals, including 25 Chinese journals and 38 English journals. The co-author rate of the included literature was 91.01 %. There were 45 articles with fund support(50.56 %). The study mainly involved ICU patients, surgical patients and elderly patients. A total of 98 keywords were extracted from Chinese literature and 59 keywords were extracted from English literature. The research hotspots of Chinese literature were divided into four groups including Braden score, prediction, pressure injury and prediction model. The research hotspots of English literature were divided into four groups including intensive care unit, pressure ulcers, nomogram and machine learning. Conclusion: The researches on the risk prediction of pressure injury based on machine learning algorithm is still in the developmental stage. In the future, the scientific and integrity of the research and design of the prediction model should be strengthened. The following can also be used as future research directions such as the construction and verification of the risk prediction model of pressure injury in different specialties, the training of machine learning algorithms and nursing education research.

Evidence summary of ocular disinfection before cataract surgery

Objective: To evaluate and summarize the best evidence of ocular disinfection before cataract surgery,and to provide evidence support for the prevention of endophthalmitis after cataract surgery. Methods: Chinese biomedical literature database and Joanna Briggs Institute(JBI), National Clinical Guidelines Database(NGC), Australian Clinical Practice Guidelines Database, New Zealand Guidelines Group, Canadian Medical Association CPG Network, UK National Institute for Health and Service Excellence(NICE),and other databases were retrieved,researches on ocular disinfection before cataract surgery including guidelines, best practice information book, evidence summary and randomized controlled trials were extracted systematically. The retrieval time is from inception to October 24, 2021. Three researchers evaluated the quality of literatures and extracted the contents. Results: A total of 7 guidelines were included. The evidence included the type of disinfectant, the concentration of disinfectant, the scope of disinfection and the time of disinfection. Conclusion:The nursing staff should choose appropriate disinfectant and use correct method to disinfect the eyes before operation, so as to reduce the bacteria community around the eyes and conjunctival sac and prevent the occurrence of endophthalmitis after operation,to ensure the safety of patients. Due to the continuous updating of evidence and humanistic nature of nursing, evidence should be selected pertinently combining with hospital environment and clinical characteristics when the evidences were applied.
