Meta analysis of exercise intervention in patients with tumor-related sarcopenia

Objective: To evaluate the effects of exercise intervention on muscle mass, muscle strength and physical function in patients with tumor-related sarcopenia patients using meta analysis. Methods: Randomized controlled trials on the effectiveness of exercise intervention on patients with tumor-related sarcopenia were retrieved from electronic databases of CNKI, Wanfang data, CBM, PubMed, Cochrane Library, CINAHL, Ovid Discovery, etc. RevMan 5.3 software was used to perform meta-analysis. Results: Eight studies were included, with 489 patients. Meta analysis results show that,exercise intervention can improve muscle mass of tumor-related sarcopenia(Skeletal Muscle Mass Index) [ SMD=0.96, 95% CI(0.68, 1.23), P<0.05], muscle mass (lean mass weight) [ SMD=0.64,95% CI(0.35, 0.94), P<0.05]; muscle strength (upper limb) [ MD=4.66, 95% CI(2.51, 6.81), P<0.05], muscle strength(leg strength) [ SMD=1.05,95% CI(0.64, 1.46), P<0.05]; Body performance (step rate)[ MD=0.50, 95% CI(0.42, 0.58), P<0.05]. Conclusion: Exercise intervention can effectively improve muscle strength, muscle mass and physical function in patients with tumor-related sarcopenia. Due to the diversity of cancer types, inconsistent outcome measurement standards, and differences in the definition of outcome indicators, the research results have high heterogeneity. So, high quality and multi-center RCT researches are needed to explore effective interventions for the treatment of tumor-related sarcopenia and to improve the homogeneity of research results.

Experience of weight management in patients with breast cancer:a qualitative meta-synthesis

Objective: To systematically summarize the experience of weight management in patients with breast cancer and comprehensively understand the needs of patients in the process of weight management, so as to provide evidence for better weight management suggestions. Methods: Pubmed、Scopus、Embase、Web of Science、CINAHL、PsycINFO、CBM、CNKI、 VIP、Wanfang were retrieved from inception to October 2021.The qualitative researches on weight management in patients with breast cancer were extracted. The quality of included studies was evaluated according to JBI critical appraisal tool for qualitative studies in Australia. The results were integrated by integrating methods. Results: A total of nine studies were included. Thirty-four complete findings were grouped according to similarities and nine new categories were formed. These categories resulted in three synthesized findings. They were effects of illness on weight management,the journey of the weight management process,factors affecting weight management. Conclusion: As a common side effect of breast cancer treatment, weight gain has a negative impact on physical and mental health of patients with breast cancer. Healthcare workers should help patients to make effective decisions according to their own conditions.

Evidence summary of ocular disinfection before cataract surgery

Objective: To evaluate and summarize the best evidence of ocular disinfection before cataract surgery,and to provide evidence support for the prevention of endophthalmitis after cataract surgery. Methods: Chinese biomedical literature database and Joanna Briggs Institute(JBI), National Clinical Guidelines Database(NGC), Australian Clinical Practice Guidelines Database, New Zealand Guidelines Group, Canadian Medical Association CPG Network, UK National Institute for Health and Service Excellence(NICE),and other databases were retrieved,researches on ocular disinfection before cataract surgery including guidelines, best practice information book, evidence summary and randomized controlled trials were extracted systematically. The retrieval time is from inception to October 24, 2021. Three researchers evaluated the quality of literatures and extracted the contents. Results: A total of 7 guidelines were included. The evidence included the type of disinfectant, the concentration of disinfectant, the scope of disinfection and the time of disinfection. Conclusion:The nursing staff should choose appropriate disinfectant and use correct method to disinfect the eyes before operation, so as to reduce the bacteria community around the eyes and conjunctival sac and prevent the occurrence of endophthalmitis after operation,to ensure the safety of patients. Due to the continuous updating of evidence and humanistic nature of nursing, evidence should be selected pertinently combining with hospital environment and clinical characteristics when the evidences were applied.

Systematic review of medication compliance of children and adolescent undergoing liver transplantation

Objective: To systematically review the influencing factors and intervention measures of medication compliance among children undergoing liver transplantation. Methods: Cochrane Library, Medline, Embase, CINAHL, CNKI, WanFang Data, CBM and VIP were retrieved from inception to February 2021. Two researchers independently conducted literature screening, data extraction and quality evaluation, and then systematic review was conducted. Results: A total of 19 studies were included, covering 7 cohort studies, 4 case-control studies, 4 cross-sectional studies and 4 quasi-experiment studies. The rate of medication noncompliance among children undergoing liver transplantation ranged from 5.6% to 59.7%. Medication adherence was affected by age, family, socioeconomic factors, mental and psychological factors, etc. Cross-age peer mentoring program and transition readiness skills, etc. can improve medication adherence. Conclusion: Elder in age, transition period from adolescence to adulthood, parents’ low education level, single-parent family, and history of abuse were influencing factors of medication compliance in children undergoing liver transplantation. Identification of compliance influencing factors and exploration of targeted intervention strategies are necessary to improve medication compliance.

Evidence summary of warning indicators of in-hospital cardiopulmonary arrest among adults

Objective: To retrieve, evaluate and summarize the best evidence of warning indicators of in-hospital cardiopulmonary arrest among adults, and to provide evidence-based basis for early intervention on in-hospital cardiopulmonary arrest. Methods: Guidelines websites and databases including the BMJ best practice, UK National Institute for Health and Clinical Excellence, International Guidelines Collaboration Network, Scottish Interscholastic Guidelines Network, China Clinical Guideline Library, CNKI, Wanfang, VIP, Cochrane Library, PubMed, Web of Science, Embase etc. were retrieved. Literatures including guidelines, expert consensus, systematic reviews with regard to in-hospital cardiopulmonary arrest among adults were included. The corresponding quality evaluation tools were used to evaluate the quality of included literatures. The retrieval time limit was from the inception of databases to June 30, 2022. Results: Five guidelines, one expert consensus, and 12 systematic reviews were included. A total of 27 pieces of the best evidence were extracted, covering 5 aspects including main vital signs, electrocardiographic, test indicators, symptoms and chief complaints,and medical history.Conclusion: This study summarized the best evidence of warning indicators of in-hospital cardiopulmonary arrest among adults. Further translation and application of these evidence based on the current situation of domestic clinical practice is needed and the suitability needs further evaluation.

Visual analysis of acupoint application in recent 10 years in China

Objective: To explore the research hotspot and trend in the field of acupoint application in the past 10 years. Methods: The relevant literature on acupoint application in CNKI database was searched. The author, institution and keyword atlas were drawn and analyzed. Results: A total of 5 426 papers were included, with a steady increase in the total number of papers, and presented two main stages. A total of 527 authors were included, the most published author was scholar Liu Lanying, and the most published institution was the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine. There were 8 keyword cluster structures. Research hotspots and frontiers mainly focus on the combined application of acupoint application and emotional nursing and other nursing methods, and the clinical application of acupoint application in different diseases. Conclusion: It is found that the future development trend of acupoint application research mainly lies in the intervention research on improving sleep quality and pain degree, strengthening the combined application of acupoint application and emotional care, and exploring the prevention of adverse reactions of acupoint application. Therefore, it is the future development direction of acupoint application to carry out multi-center, large sample, deep related direction research and internal mechanism exploration.

Summary of the best evidence of perioperative nutritional management for  patients with hepatobiliary and pancreatic diseases

Objective: To select and obtain relevant evidence for perioperative nutritional management for patients with hepatobiliary and pancreatic diseases, and to summarize the best evidence. Methods: The British Journal of Medicine ( BMJ ) , National Guideline Clearinghouse (NGC) , National Institute for Health and Care Excellence(NICE) , Joanna Briggs Institute(JBI) evidence-based Health Care Center Database, the Cochrane Library, PubMed, CBM, CNKI and Wanfang Database on perioperative nutritional support for patients with hepatobiliary and pancreatic diseases were retrieved. All evidence of patients with perioperative nutritional management for patients with hepatobiliary and pancreatic diseases included highest clinical decision support, guidelines, systematic evaluation, evidence summaries, expert consensus and recommendations, etc. The time limit for the retrieval was from the inception of databases to Dec 30, 2022. Two to four researchers independently evaluated the literature and extracted the documents that met the standards according to the judgment of professionals. Results: A total of 7 articles were included, including 5 guidelines, 2 expert consensus and recommendations. The best evidence was summarized from six aspects. They were nutritional assessment, preoperative nutritional support , postoperative nutritional support , post -discharge nutritional support , nutritional management and nutritional preparation selection in total of 25 items . Conclusion: This study summarizes the best evidence for perioperative nutritional management for patients with hepatobiliary and pancreatic diseases, and provides evidence-based evidence for perioperative nutritional management for patients with hepatobiliary and pancreatic diseases. Medical personnel should apply evidence in combination with the specific conditions of their own hospitals and departments, and use evidence in a targeted manner to reduce the incidence of malnutrition in patients undergoing hepatobiliary and pancreatic surgery.

Care experience of the primary caregiver of children with epilepsy:a study  of qualitative Meta-synthesis

Objective: To systematically evaluate the care experience of the primary caregivers of children with epilepsy, and to improve the basis for making care plans that meet the needs of caregivers. Methods: PubMed, Embase, the Cochrane Library, CNKI, Wanfang and other databases were searched from inception of databases to April 14, 2022. The Australian JBI quality evaluation standard(2016) and the aggregative integration method were used to evaluate the literature quality and integrate the results. Results: A total of 16 literatures were included, 65 results were extracted, 11 new categories were summarized, and 3 integrated results were synthesized. Integration outcome 1 was the negative characteristics of the disease and the enormous physical and mental pressure of caregivers which had an impact on family functions and daily life, and promoted caregivers to over-protect their children and changed their values; Integration outcome 2 was that caregivers needed support from family members, had limited knowledge of disease and care, and craved social and medical support; Integration outcome 3 was that caregivers actively adjusted themselves to cope with illness and grew from illness care. Conclusion: Medical staffs should pay attention to the care needs and psychological experience of caregivers of epileptic children, provide multi-directional support, assist them to play the role of caregivers, and ensure the healthy growth of children.
