基于奥马哈问题分类系统的胃癌术后化疗患者随访清单的制订及初步应用

Development and preliminary application of a follow-up checklist for gastric cancer postoperative chemotherapy patients based on the Omaha Problem Classification System

摘要: 目的:制订科学、规范的胃癌术后化疗患者随访清单,为临床开展随访工作提供工具支持。方法:通过文献回顾、半结构化访谈及小组讨论,制订胃癌术后化疗患者随访清单初稿,于2024年12月至2025年3月采用德尔菲法进行2轮专家函询,依据专家意见修改完善清单内容,形成随访清单终稿。采用便利抽样法,于2025年4月,使用随访清单对21例胃癌术后化疗患者进行随访,评估随访时长,了解护士、患者对随访清单的使用意见。结果:2轮专家函询的积极系数分别为88.89%、100%,专家权威系数分别为0.90、0.91,肯德尔和谐系数分别为0.249和0.270(P<0.001)。最终形成的胃癌术后化疗患者随访清单包括4个一级条目、17个二级条目、43个三级条目。对胃癌术后化疗患者的随访时长为(22.86±3.18)min,护士和患者均认为随访清单内容全面、实用。结论:胃癌术后化疗患者随访清单内容全面,具有科学性、可靠性和适用性,为护理人员开展高质量随访工作提供科学的工具支持。

Abstract: Objective: To establish a scientific and standardized follow-up checklist for gastric cancer postoperative chemotherapy patients, thereby providing a practical tool to support clinical follow-up work. Methods: A preliminary version of the follow-up checklist was developed through literature review, semi-structure interviews, and group discussions. From December 2024 to March 2025, two rounds of Delphi expert consultations were conducted. The checklist content was revised and finalized based on expert feedback. Employing a convenience sampling method, a follow-up assessment was conducted in April 2025 using standardized checklists to evaluate 21 gastric cancer patients undergoing postoperative chemotherapy. The study measured follow-up duration and collected feedback from both nurses and patients regarding the checklist implementation. Results: The response rates for the two rounds of expert consultations were 88.89% and 100%, respectively. The expert authority coefficients were 0.90 and 0.91, and the Kendall's W coefficients were 0.249 and 0.270 (P<0.001). The finalized follow-up checklist for gastric cancer patients undergoing postoperative chemotherapy includes 4 first-level items, 17 second-level items, and 43 third-level items. The average follow-up duration for these patients was (22.86±3.18) minutes. Both nurses and patients perceived the checklist to be comprehensive and practical in its content. Conclusion: The follow-up checklist for gastric cancer postoperative chemotherapy patients demonstrates comprehensive content, along with scientific rigor, reliability, and applicability. It serves as a scientifically grounded tool to support nursing staff in conducting high-quality follow-up care.
