基于积极心理学视域的中青年脑卒中患者重返工作自我效能水平及影响因素研究

摘要: 目的:调查中青年脑卒中患者重返工作自我效能水平,并分析其影响因素。方法:采用便利抽样法,选取 2022 年 9 月至2024 年 1 月某三级甲等医院收治的中青年脑卒中患者为研究对象。采用一般资料调查表、重返工作自我效能量表、自尊量表、心理一致感量表、Herth 希望指数量表进行调查。采用 Pearson 相关分析法分析重返工作自我效能与积极心理学要素(自尊、心理一致感、希望)的相关性;采用多元线性回归分析法分析重返工作自我效能的影响因素。结果:172 例中青年脑卒中患者的重返工作自我效能评分(4.12±0.44)分,自尊评分(25.67±4.96)分,心理一致感评分(59.83±10.11)分,希望评分(36.06±5.21)分。重返工作自我效能与自尊(r=0.715)、心理一致感(r=0.689)、希望水平(r=0.785)均呈正相关(P<0.01)。多元线性回归分析显示,改良 Rankin评分、自尊、心理一致感、希望是中青年脑卒中患者重返工作自我效能的影响因素,可共同解释总变异的 74.2%。结论:中青年脑卒中患者重返工作自我效能水平有待提高,积极心理学要素(自尊、心理一致感、希望)与改良 Rankin 评分共同影响重返工作自我效能水平。临床护理人员可加强对积极心理学要素的关注,同时根据患者特征采取针对性措施提高患者重返工作自我效能水平,促进患者重返工作进程。

Abstract: Objective: To investigate the level of return-to-work self-efficacy in young and middle-aged stroke patients and analyze its influencing factors. Methods: A convenience sampling method was adopted to recruit young and middle-aged stroke patients who were admitted to a tertiary grade A hospital between September 2022 and January 2024. Data were collected using a general information questionnaire, the Return-to-Work Self-Efficacy Questionnaire (RTW-SE), the Self-Esteem Scale, the Sense of Coherence-13 (SOC-13), and the Herth Hope Index Scale (HHI). Pearson correlation analysis as conducted to explore the relationships between return-to-work self-efficacy and positive psychological factors (self-esteem, sense of coherence, and hope). Multiple linear regression analysis was performed to identify the influencing factors of return-to-work self-efficacy. Results: Among 172 young and middle-aged stroke patients, the score of return-to-work self-efficacy was 4.12±0.44, the self-esteem score was 25.67±4.96, the sense of coherence score was 59.83±10.11, and the hope score was 36.06 ± 5.21. Return-to-work self-efficacy was positively correlated with self-esteem (r=0.715), sense of coherence (r=0.689), and hope level (r=0.785) (all P<0.01). Multiple linear regression analysis showed that modified Rankin Scale score, self-esteem, sense of coherence, and hope were influencing factors of return-to-work self-efficacy in young and middle-aged stroke patients (all P<0.05), collectively explaining 74.2% of the total variance. Conclusion: The level of return-to-work self-efficacy among young and middle-aged stroke patients requires further improvement. Positive psychological factors (self-esteem, sense of coherence, and hope), along with the mRS score, collectively influence return-to-work self-efficacy. Therefore, clinical nurses should strengthen their focus on positive psychological factors and develop tailored interventions based on individual patient characteristics to enhance return-to-work self-efficacy and facilitate the return-to-work process in this population.
