急诊科护士创伤后应激障碍发生率的 Meta 分析

Incidence of post-traumatic stress disorder among emergency department nurses: a meta-analysis

摘要: 目的:系统评价急诊科护士创伤后应激障碍(post-traumatic stress disorder,PTSD)的发生率。方法:计算机检索 8 个英文及 4 个中文数据库有关急诊科护士 PTSD 发生率的流行病学研究,检索时限均为建库至 2022 年 10 月。由 2 名研究者严格按照纳排标准独立进行文献筛选和数据提取,并应用美国卫生保健质量和研究机构量表(AHRQ)评价横断面研究质量。采用 Stata 11.0 软件进行 Meta 分析。结果:共纳入 14 篇文献,共包括 2 370 名急诊科护士,其中 PTSD 阳性 612 名。急诊科护士 PTSD 发生率为 27.0% [95%CI(16.6%,38.8%)]。亚组分析结果显示:急诊科男护士与女护士 PTSD 发生率分别为 44.6%和 30.8%,国内与国外急诊科护士 PTSD 发生率分别为 27.8%和 25.2%,在检索的 6 种 PTSD 评估工具中,采用密西西比创伤后应激障碍量表(平民版)诊断的急诊科护士 PTSD 发生率最高,为 83.21%。结论:当前证据显示,男性、国内、创伤后应激障碍诊断标准为密西西比创伤后应激障碍量表(平民版)的急诊科护士 PTSD 发生率高。因为纳入研究数量和质量的限制,尚待更多高质量流行病学研究予以验证上述结论。

Abstract: Objective: To systematically evaluate the incidence of post-traumatic stress disorder(PTSD) among emergency department nurses. Methods: Eight English and four Chinese databases were searched for epidemiological studies related to the incidence of PTSD among emergency department nurses. Searches were conducted from database inception to October 2022. Literature screening and data extraction were completed by 2 researchers independently according to the inclusion and exclusion criteria, and cross-sectional study quality was evaluated by applying the AHRQ scale. Meta - analysis was performed with Stata 11.0 software. Results: A total of 14 articles were included, including 2 370 emergency department nurses, of which 612 were positive for PTSD. According to subgroup analysis, the incidence of PTSD in male and female emergency department nurses was 44.6% and 30.8%, respectively. The incidence of PTSD among emergency department nurses in China was 27.8%, while the incidence of PTSD among emergency department nurses abroad was 25.2% . Among the 6 PTSD assessment tools retrieved, emergency department nurses diagnosed with the Mississippi Posttraumatic Stress Disorder Scale (civilian version) had the highest incidence of PTSD, at 83.21%. Conclusion: Current evidence shows that the incidence of PTSD among emergency department nurses was higher among emergency department nurses who were male, whose study site was China, and diagnosed with the Civilian Mississippi scale. Due to the limitations of the number and quality of included studies, the above conclusions still need to be verified by more highquality epidemiological studies.
