The relationship between health literacy, self-neglect, and social support in elderly breast cancer patients

老年乳腺癌患者健康素养与自我忽视、社会支持关系的研究

Abstract: Objective: To explore the current status of health literacy in elderly breast cancer patients and analyze the relationship between health literacy, self-neglect, and social support. Methods: Using a convenient sampling method, general information questionnaire, chronic disease health literacy scale, elderly self -neglect assessment scale, and perceived social support scale were used to conduct the investigation. A total of 175 elderly breast cancer patients in a grade A tertiary hospital in Tianjin were included. Results: The total score of health literacy in elderly breast cancer patients was 100.13±21.30, and 28% of elderly breast cancer patients lacked health literacy. Pearson correlation analysis showed that health literacy was negatively correlated with self-neglect and its various dimensions (r=-0.538~-0.390, P<0.001), and positively correlated with social support and its various dimensions (r=0.177~0.682, P<0.05). Multiple step-wise regression analysis showed that age, occupation, monthly per capita income, the dimension of living environment and hygiene in self-neglect, and the dimension of family support in social support were the main influencing factors of health literacy in elderly breast cancer patients. Conclusion: Although the health literacy level of elderly breast cancer patients is adequate, there are some patients who lack health literacy. Clinical workers should analyze the influencing factors of patients′ health literacy, take corresponding intervention measures, and improve the health literacy level of patients by reducing the self-neglect level and increasing the social support level.

摘要: 目的:探讨老年乳腺癌患者健康素养现状,并分析其与自我忽视、社会支持的关系。方法:采用便利抽样的方法,采用一般资料问卷、慢性病健康素养量表、老年人自我忽视评估量表、领悟社会支持量表对天津市某三级甲等医院 175 例老年乳腺癌患者进行调查分析。结果:老年乳腺癌患者健康素养总分为(100.13±21.30)分,其中 28%的老年乳腺癌患者缺乏健康素养。Pearson 相关分析显示,健康素养与自我忽视及其各维度呈负相关(r=-0.538~-0.390,P<0.001),与社会支持及其各维度呈正相关(r=0.177~0.682,P<0.05)。多元逐步回归分析显示,年龄、职业、人均月收入、自我忽视中的居住环境卫生维度及社会支持中的家庭支持维度是老年乳腺癌患者健康素养的主要影响因素(P<0.05)。 结论:老年乳腺癌患者健康素养水平充足,但仍有部分患者缺乏健康素养,临床工作者应分析患者健康素养的影响因素,采取相应的干预措施,并通过降低患者自我忽视水平及提高社会支持水平,提高患者健康素养水平。
