Analysis of the compliance of external medication and influencing factors of young and middle-aged patients with psoriasis

中青年银屑病患者外用药治疗依从性及影响因素分析

Abstract: Objective: To explore the status quo and influencing factors among young and middle-aged patients with psoriasis who receive maintenance treatment with topical medications according to the prescribed course, and to provide a reference for further formulation of targeted nursing measures. Methods: Using the convenience sampling method, the young and middle-aged psoriasis patients who visited the dermatology department of a tertiary grade A hospital in Tianjin from October 1, 2022 to October 20, 2023 were selected as the research subjects. General information questionnaire, Morisky medication compliance evaluation scale, social appearance anxiety scale (SAAS) and social support rating scale were used for the investigation. Logistic regression model was employed to analyze the influencing factors. Results: Among the subjects, 201 (62.42%) had good compliance. Multivariate analysis showed that social appearance anxiety score, psoriasis comorbidities, types of combined drugs and adverse drug reactions were the main influencing factors for the compliance of young and middle-aged patients with psoriasis. Conclusion: The medication compliance of psoriasis in young and middle-aged patients is at a moderate level. Attention should be paid to patients with low level of social appearance anxiety, more comorbidities, more types of combined drugs and severe adverse drug reactions. Targeted measures such as strengthening health education, continuous follow-up and reducing adverse drug reactions should be taken to improve the compliance of external drugs and promote the therapeutic effect.

摘要: 目的:探究中青年银屑病患者按照疗程接受外用药维持治疗的依从性现状及其影响因素,为进一步制定针对性护理措施提供参考。方法:采用方便抽样法,选取 2022 年 10 月 1 日至 2023 年 10 月 20 日在天津市某三级甲等医院皮肤科就诊的中青年银屑病患者为研究对象。采用一般资料调查表、8-条目 Morisky 用药依从性评价表、社交外表焦虑量表、社会支持评定量表进行调查。 采用多元线性回归模型分析影响因素。结果:调查对象中有 201 例(62.42%)依从性较好。多元线性回归分析显示,社交外表焦虑得分、银屑病共病、联合用药种类、药物不良反应为中青年银屑病患者外用药治疗依从性的主要影响因素。结论:中青年银屑病患者外用药治疗依从性处于中等水平,应当重点关注社交外表焦虑水平较低、合并共病较多、联合用药种类较多以及药物不良反应严重的患者,通过加强健康教育、持续随访追踪、减少不良反应等针对性措施提高外用药治疗依从性,促进治疗效果。
