Symptom cluster and network analysis during the treatment of lymphoma patients

淋巴瘤患者治疗期间的症状群及网络分析

Abstract: Objective: By using network analysis methods, a concurrent symptom network for lymphoma patients was constructed to identify the symptom clusters and to explore the core symptoms within the symptom networks as well as the bridge symptoms that connect different symptom clusters in the patients. Methods: Patients admitted to the lymphoma department of a tertiary grade A hospital in Tianjin by using the convenient sampling method. The investigation was conducted using the general information questionnaire and lymphoma patient symptom assessment scale. The network of 23 symptoms was conducted by using R4.3.2, and the symptom groups were identified through the network clustering algorithm. Centrality indicators and bridge centrality indicators were calculated to determine the core symptoms and bridge symptoms. Results: The patients had 5 symptom clusters, including gastrointestinal tract symptom cluster, chemotherapy side-effect symptom cluster, skin-metabolism symptom cluster, systemic symptom cluster of the disease, and mood-sleep symptom cluster. Chemotherapy side effects were the most severe symptom cluster for patients. In the symptom networks, anxiety (rS= 1.23), decreased appetite (rS=1.20), and hiccups (rS=1.14) were the three symptoms with the highest strength, while anxiety (rBS=8.83), sadness (rBS=8.73), and hiccups (rBS=8.34) were the three symptoms with the highest bridge strength. Conclusion: Lymphoma patients have multiple symptom clusters during treatment, and anxiety is the most central symptom in the symptom networks, as well as a central bridge symptom that connects different symptom clusters. It is recommended that healthcare professionals focus assessment and intervention on the core and bridge symptoms during treatment, thereby improving the efficiency and precision of symptom management for lymphoma patients.

摘要: 目的:通过网络分析法构建淋巴瘤患者的同期症状网络,识别患者存在的症状群,探索症状网络中的核心症状以及连接不同症状群的桥梁症状。方法:采用便利抽样法,选取天津市某三级甲等医院淋巴瘤内科收治的患者为研究对象,采用一般资料调查表和淋巴瘤患者症状评估量表进行调查。运用 R4.3.2 构建 23 种症状的网络,通过网络聚类算法识别症状群,计算中心性指标和桥梁中心性指标确定其核心症状和桥梁症状。结果:淋巴瘤患者存在 5 大症状群,包括胃肠道症状群、化疗不良反应症状群、皮肤-代谢症状群、疾病全身症状群和情绪-睡眠症状群,化疗不良反应是患者最严重的症状群。在症状网络中,焦虑(rS=1.23)、食欲下降(rS=1.20)和呃逆(rS=1.14)是强度最大的 3 种症状,而焦虑(rBS=8.83)、悲伤(rBS=8.73)和呃逆(rBS=8.34)是桥梁强度最大的3 种症状。结论:淋巴瘤患者在治疗期间存在多个症状群,焦虑是其症状网络中最核心的症状,同时也是不同症状群相互作用的核心桥梁症状。建议医护人员针对治疗期间核心症状和桥梁症状进行重点评估和干预,进而提高淋巴瘤患者症状管理的效率和精准度。
