ICU患者肠内营养性腹泻影响因素分析

Study on influencing factors of enteral nutritional diarrhea in ICU patients

摘要:  

目的:探讨ICU患者肠内营养性腹泻的影响因素,为制定护理对策提供依据。方法:选取ICU住院患者116例,按其发生肠内营养性腹泻与否分成腹泻组62例,非腹泻组54例。对两组患者的疾病严重程度、排便、用药情况进行调查,并对数据进行单因素和多因素Logistic回归分析,找出发生肠内营养性腹泻的原因。结果:Logistic回归分析发现,年龄、应用抗生素、胃动力学药、抗酸药、钾剂、益生菌是肠内营养性腹泻的影响因素,其中益生菌应用是ICU肠内营养性腹泻患者的保护性因素,OR值为0.865,年龄、应用抗生素、胃动力学药、抗酸药、钾剂是肠内营养性腹泻的危险因素,OR值均大于1。结论:年龄、应用抗生素、胃动力学药、抗酸药、钾剂是ICU肠内营养性腹泻的主要危险因素,临床应尽量避免危险因素以提高营养支持的耐受性。

Abstract:  

Objective: To explore the causes of enteral nutritional diarrhea in ICU patients, and to provide a basis for the formulation of nursing countermeasures. Methods: 116 hospitalized patients with ICU were selected and 62 cases were divided into diarrhea group and 54 cases of non-diarrhea group. The severity, defecation and drug use of two groups of patients were investigated, and univariate and multivariate Logistic regression analysis was performed to find out the cause of enteral nutrition diarrhea. Results: Logistic regression analysis showed that age, application of antibiotics, gastric dynamic agents, antacids, potassium and probiotics were associated with enteral nutritional diarrhea(P<0.05). The application of probiotics was a protective factor in patients with ICU enteral nutrition diarrhea. The OR value was 0.865. Age, antibiotics, gastric dynamic agents, antacids and potassium agents were risk factors for enteral nutritional diarrhea, and OR values were all greater than 1. Conclusion: Age, application of antibiotics, gastric dynamic agents, antacids and potassium agents are the main risk factors for ICU enteral nutrition diarrhea, and clinical risk factors should be avoided to improve the tolerance of nutrition support.
