康复期乳腺癌患者健康促进行为现状及影响因素研究

Study of health-promoting behaviors and its influencing factors in breast cancer patients during rehabilitation

摘要: 目的:调查康复期间乳腺癌患者健康促进行为的现状并分析其影响因素。 方法:选取275例康复期乳腺癌患者,采用一般资料调查表、健康促进生活型态量表Ⅱ、Connor-Davidson心理弹性量表、简易心理状况评定量表以及领悟社会支持量表进行横断面调查。采用多元逐步回归分析健康促进行为的影响因素。结果: 共收回有效问卷257份。257例康复期乳腺癌患者的健康促进行为总分为(107.86±19.32)分,各维度中得分最低的3项由低到高分别是躯体活动、健康责任、压力管理;回归分析显示,内分泌治疗、运动频次、心理弹性、社会支持维度中家庭支持、朋友支持以及心理健康状况是影响健康促进行为的主要因素(调整后R2=0.462,F=25.823,P<0.001)。结论:康复期乳腺癌患者健康促进行为水平较低,医护人员需建立长期随访机制和便捷的咨询服务,为患者制定科学性、个体化的运动方案,同时加强患者心理健康状况评估与干预,提升患者心理弹性,鼓励家庭成员积极参与,将有助于提升患者健康促进行为。

Abstract: Objective: To investigate the level of health-promoting behaviors and analyze its influencing factors in breast cancer patients during rehabilitation. Methods: A total of 275 patients with breast cancer during rehabilitation were investigated using the questionnaires including the general information scale, the health-promoting lifestyle profileⅡ, Connor-Davidson resilience scale, Kessler psychological distress scale and perceived social support survey. The influencing factors of health-promoting behaviors were analyzed using multiple stepwise regression. Results: A total of 257 valid questionnaires were collected. The total score of health-promoting behaviors of 257 patients was 107.86±19.32. The 3 lowest scores in each dimension from low to high were physical activity, health responsibility and stress management. Multiple stepwise regression showed that endocrine therapy, daily exercise frequency, resilience, family support and friend support dimension of social support, mental health were the main factors affecting health-promoting behaviors, with 46.2% variable coefficient(F=25.823, P<0.001). Conclusion: Breast cancer patients during rehabilitation have low level of health-promoting behaviors. Healthcare providers are suggested to establish long-term follow-up and convenient consultation services, formulate scientific and targeted exercise plans, strengthen patients’ mental health assessment and intervention, improve patients’ resilience, and encourage family members to participate actively, which is significant for promoting patients’ healthy lifestyle behaviors.
