产妇硬膜外镇痛分娩体验质性研究的系统评价

The experience of maternal epidural analgesia delivery: a systematic review of qualitative studies

摘要: 目的:系统评价有关产妇硬膜外镇痛分娩体验的质性研究,为了解硬膜外镇痛分娩产妇的分娩体验、改善产科硬膜外镇痛分娩 医疗服务提供参考依据。 方法:检索 PubMed、Web of Science、PsycINFO、Embase、中国知网、中国生物医学文献数据库、万方、维普数据 库中关于产妇硬膜外分娩镇痛体验的质性研究, 检索时限从建库至 2022 年 6 月。 采用澳大利亚乔安娜布里格斯研究所(Joanna Briggs Institute,JBI)循证卫生保健中心质性研究质量评价标准(2016 版)对纳入研究进行质量评价,并采用汇集性整合方法整合结果。 结果:共纳入 14 篇文献,提炼 46 个研究结果,归纳为 7 个类别,综合成 3 个整合结果,分别为硬膜外镇痛分娩产妇的决策经历、硬 膜外镇痛分娩产妇的身心体验、硬膜外镇痛分娩产妇存在知识与情感的需求。 结论:护理人员应充分关注产妇硬膜外镇痛分娩决策 时的困境、重视积极分娩体验、满足知识与情感需求,增强分娩体验满意度,从而提高护理质量。

Abstract: Objective: To systematically review qualitative studies on the experience of maternal epidural delivery analgesia, so as to provide reference basis for clinical improvement of labor analgesia management and obstetric medical service . Methods: PubMed, Web of Science, PsycINFO, Embase, CNKI, CBM, WanFang, VIP databases were retrieved from inception to June 2022,and literatures on qualitative studies of the maternal epidural analgesia delivery were extracted. The JBI critical appraisal tool for qualitative studies in Australia (2016 edition) was used to evaluate the quality of studies.The results were syn-the sized by using integration methods. Results: A total of 14 studies were included,and 46 research results were extracted. 3 integrated results were grouped from 7 categories according to their similarities, covering the decision making experience of maternal epidural analgesia delivery, the physical and mental experience of maternal epidural analgesia delivery, maternal knowledge and emotional needs. Conclusion: Nursing staff should fully focus on the plight of maternal epidural analgesia childbirth decisions, attaches great importance to the active birth experience, meet the demand of knowledge and the emotion, and strengthen birth experience.
