中青年原发性肝癌伴 HBV 阳性患者配偶社会疏离体验的质性研究

Social isolation experience of spouses of young and middle-aged patients with hepatocellular carcinoma combined with HBV positive:a qualitative study

摘要: 目的:明确中青年原发性肝癌伴 HBV 阳性患者配偶的社会疏离体验。 方法:采用描述性质性研究,通过目的性抽样法, 于 2021 年 11 月至 2022 年 3 月选取天津市某三级甲等医院肝胆肿瘤科 15 名中青年原发性肝癌伴 HBV 阳性患者的配偶进行半结构式访谈,采用 Colaizzi7 步分析法分析资料。结果:中青年原发性肝癌伴 HBV 阳性患者的配偶经历社会疏离的真实体验共提炼出 2 个主题及 7 个副主题:社会疏离的表现(孤独无力感、社交回避)、社会疏离的原因(支持力量薄弱、照顾角色需求、公众歧视、自我歧视、利他心理)。 结论:中青年原发性肝癌伴 HBV 阳性患者配偶在患者确诊疾病后存在社会疏离, 多呈现消极体验。 建议护理人员关注此类人群的消极疏离体验,正确引导他们对 HBV 及癌症等的态度和行为,降低自我歧视、公众歧视及社交回避;同时充分利用家庭支持来协助肝癌患者配偶降低社会疏离感。

Abstract: Objective: To explore the experiences of spouses whose husband or wife was diagnosed with hepatocellular carcinoma combined with HBV positive. Methods: A qualitative descriptive design was adopted. A total of 15 spouses were purposefully recruited and interviewed using a semi-structured interview guide in a Class A tertiary cancer hospital in Tianjin, China. The Colaizzi seven-step analysis method was used to analyze the data. Results: Two themes and seven sub-themes were extracted from the data, including manifestations of social isolation (loneliness and powerless, social avoidance); reasons for social isolation(little support, caring role needs, public discrimination, self-discrimination and altruism). Conclusion: Spouses of young and middle-aged cancer patients who were diagnosed with hepatocellular carcinoma combined with HBV positive experience social isolation in the process of providing care to the patients. Clinical nursesare strongly advised to pay attention to this population and take actions to relieve their social isolation, guiding them to hold positive attitudes towards HBV and hepatocellular carcinoma, reducing self-iscrimination, public discrimination and social avoidance. At the same time, close family members are expected to continuously support the spouses of cancer patients who were diagnosed with hepatocellular carcinoma combined with HBV positive.
