The psychological experiences of breast cancer patients after mastectomy: a qualitative Meta-synthesis

摘要: 目的:系统评价乳腺癌患者乳房切除术后心理体验的质性研究,为提高该人群心理健康和建立支持体系提供参考。方法:检索中外数据库中关于乳腺癌患者乳房切除术后心理体验的质性研究,检索时限从建库至 2022 年 12 月 31 日。采用 JBI 循证卫生保健中心质性研究质量评价标准对文献质量进行评价,采用汇集性整合法对结果进行 Meta 整合。结果:共纳入 13 篇文献,提炼出53个研究结果,归纳形成 10 个新类别,综合为 3 个整合结果。分别为患者术后身体形象恶化、心理健康及性生活受到影响;采取创伤后成长、乳房再造等积极正面的应对策略;患者面临一系列困扰和挑战。结论:乳房切除术影响乳腺癌患者的身心健康及社会交往,患者存在多方面的心理体验。医务人员在今后工作中需重视患者对乳腺癌切除术的认知与体验,鼓励患者积极应对,并利用相应的心理干预措施减轻或缓解乳房切除术带给患者的困扰,完善其社会支持,提高其生活质量。

Abstract: Objective: A systematic review of qualitative studies on the psychological experiences of breast cancer patients after mastectomy, providing a reference for improving mental health and establishing support systems for this population. Methods: A systematic search was conducted in both Chinese and foreign databases for qualitative studies on the psychological experiences of breast cancer patients after mastectomy, with a retrieval period from the inception of the databases to December 31,2022. The quality of the included studies was assessed using the Joanna Briggs Institute (JBI) evidence-based health center′s quality appraisal criteria for qualitative research, and the results were synthesized using a Meta-aggregation approach. Results: A total of 13 studies were included, from which 53 research findings were extracted and categorized into 10 new categories. These were synthesized into 3 integrated results, they were deterioration in body image, impacts on psychological health and sexual life, adoption of positive coping strategies such as post-traumatic growth and breast reconstruction, and a range of challenges and difficulties faced by patients. Conclusion: Mastectomy affects the physical and mental health, as well as the social interactions of breast cancer patients, leading to a variety of psychological experiences. Healthcare professionals need to pay attention to patients′ perceptions and experiences of mastectomy, encourage patients to cope with questions positively, and implement appropriate psychological interventions to alleviate or mitigate the distress caused by the mastectomy. Enhancing social support and improving the quality of life for these patients should be a focus.
