Decision making experience of prophylactic mastectomy in breast cancer patients and people with high risk: a qualitative Meta-synthesis

摘要: 目的:整合乳腺癌患者及高发人群关于预防性乳房切除决策体验的相关研究,以期全面了解关于预防性乳房切除决策体验和相关影响因素。方法:系统检索数据库 PubMed、Scopus、Embase、Web of Science、CINAHL、PsycINFO、CBM、CNKI、VIP、Wanfang,检 索时限为建库至 2022 年 10 月。搜集有关乳腺癌患者及高发人群关于预防性乳房切除决策体验的质性研究文献,对最终纳入的文 献进行质量评价,并用汇集性整合的方法进行整合。结果:最终纳入 12 篇文献,提取 51 个完整明确的研究结果,归纳为 13 个新类别,形成 4 个整合结果。结论:乳腺癌患者及高发人群对预防性乳房切除的态度多样,并且存在患癌恐惧、对乳房对称性的担忧、随访筛查的负担以及缺乏信息等相关支持。医护人员了解影响女性选择预防性乳房切除的相关因素,帮助患者知情决策。

Abstract: t Objective: Systemically summarize decision making experience of prophylactic mastectomy in breast cancer patients and high -risk populations and fully understand their experience. Methods: The databases PubMed, Scopus, Embase, Web of science, CINAHL, psycINFO, CBM, CNKI, VIP and Wanfang were retrieved by the system from inception to October 2022, to collect the qualitative research articles about breast cancer patients and high -risk populations on the decision-making experience of prophylactic mastectomy. The final included literature was evaluated for quality and the results were integrated by integrating methods. Results: A total of 12 studies were included and 51 complete findings were grouped according to similarities to form 13 new categories. These categories resulted in 4 synthesized findings. Conclusion: Breast cancer patients and people with high risk of breast cancer have a variety of attitudes towards prophylactic mastectomy. They suffered from fear and anxiety, as well as lack of information and support, both before and after surgery. Medical staffs understand the factors influencing them with the aim of facilitating informed decision-making.
